Bechterew Test -
The Bechterew test, also known as the Sitting Straight Leg Raise Test, is a neurological examination technique used to assess nerve root irritation and sciatica in seated patients. It’s similar to the slump test.

It was named after the Russian neurologist Vladimir Bechterew, this test provides a practical alternative to the traditional straight leg raise test, particularly useful when examining patients who have difficulty lying supine or when a quick screening assessment is needed.

The Bechterew test emerged as a modification of more comprehensive neural tension tests, designed to evaluate nerve root compression and irritation in the lumbar spine. Its significance lies in its ability to reproduce radicular symptoms through mechanical stress on the neural structures, particularly the sciatic nerve and its roots (L4-S3). This test is valuable in identifying nerve root pathology associated with lumbar disc herniation, spinal stenosis, and other conditions causing neural impingement.

Flexion vs Extension Bias
🔹 Flexion Bias → Symptoms reduce in sitting, forward bending, or knees-to-chest. Common in spinal stenosis & facet arthritis (usually older patients).

🔹 Extension Bias → Symptoms reduce in standing, walking, or back-arching (McKenzie extensions). Common in disc herniation (often younger patients).

👉 Knowing a patient’s directional preference helps us prescribe the right movement strategy and avoid aggravating exercises.
Because in spine rehab… direction matters!

Cloward sign and referred pain by deep tissue stimulation of the cervical spine?

Stimulation in the midline of the anterior aspect of the disc produced pain between the shoulders in the middle of the back.

Cloward described that pain from the C6-7 disc was felt in the inferior angle of the scapula.

Pain from the C5-6 disc was felt in the center of the medial scapular border.

Pain from C4-5 disc was experienced in the region of the spine and superior angle.

Pain from the C3-4 disc was referred to the C7 spinous process and the posterior border of the trapezius muscle.

Cloward also documented that when stimulating patients with posterolateral disc protrusions, the referral patterns were found to be more intense than when stimulating the anterior aspect of the disc and were found to spread from the vertebral border of the scapula out to the shoulder and upper arm as far as the elbow.

Midline posterior disc protrusions were found to refer pain to a confined area overlying the fifth cervical to the second thoracic spinous processes near the midline, with upper discs more cephalad and lower discs more caudad.

1. Pendular mobilization: 20 circles in one direction, 20 in the other. Does not help recovery, but improves pain.

2. Passive stretching: with hands or forearms resting on a table, slowly move backwards without moving them, lowering the buttocks. Repeat 5 to 10 times.

3. Internal rotation stretch: sitting or standing, place the hand of the affected arm on the healthy shoulder, with the arm crossed over the chest. Hold for 3 seconds. Repeat 5 times.

4. Shoulder roll: 20 circles in one direction, 20 in the other. Does not help recovery, but improves pain.

5. Forward stretch: can be performed lying down, sitting up or standing. With hands joined, the non-diseased arm supports the diseased arm. Do 10 to 20 times per session. This is the most important stretch for recovery. Perform with each exercise until recovery.

6. External-rotation stretching: place your hands behind your head (on your forehead in the early stages if you feel too much pain) and spread your elbows until you feel comfortable. Can be performed lying down, sitting or standing. Repeat 10 times.
