Laser iridoplasty has left its mark in a patient who apparently underwent the procedure at 18 years old for plateau iris and later had a trabeculectomy. The fellow eye is now uncontrolled, but surely lens extraction would be the better option?

Ripasudil eye drops ( Rho Kinase or Rock Inhibitors ) Induced Reticular Corneal Oedema - also described as Honeycomb Corneal Oedema.

Similar keratopathy has also been described for other RoCK inhibitors like Netrasudil recently .

Fortunately it’s fairly reversible once you stop the drops .

So if you see any such pattern in patients who are on treatment for glaucoma with RoCK inhibitors or for treatment of Corneal Oedema by Rock Inhibitors - keep this in mind ✨

Cyst-em of a Down 🤘🏼

This 27yo female was referred in for an iris lesion found while the referring OD was checking Van Herrick angles (Awesome catch, Doc).
 
Primary iris cysts are believed to be congenital and occur sporadically. Iris pigment epithelium cysts are the most common form found in adults. Located underneath the iris, these cysts push the iris forward, creating a dome-shaped surface.  These cysts are idiopathic and most commonly detected in early adulthood.  

With these lesions, the most crucial differential is an Iris melanoma. Iris melanomas most commonly appear as a brown lesion arising from the stroma and will maintain stable contour even after dilation. Presence of intrinsic vessels within the lesion are suggestive of a solid tumor rather than a benign cyst. Other signs suggestive of a malignant lesion are prom­inent episcleral blood vessels, unexplained acute increase in intraocular pres­sure, or iris heterochromia.
 
Despite the high-resolution imaging of Anterior Segment-OCT, ultrasound biomicroscopy remains the gold-standard imaging due to ability to better characterize the interior of the cystic structures.
 
Iris cysts that are stable and not causing symptoms or secondary complications such as iritis, angle-closure, pigment dispersion syndrome, focal cataract, or corneal edema require no treatment but should be followed regularly.
