Aminoglycoside inhalational therapy: a potential pitfall of antimicrobial stewardship in outpatient settings

our study revealed that aminoglycosides were the most frequently prescribed PABs in outpatient settings, especially in children, in Japan. It was hypothesized that aminoglycosides were used as inhalational therapy. Thus, aminoglycoside inhalational therapy may be a pitfall of ASPs in outpatient settings and further studies evaluating the reasons behind aminoglycoside use in outpatient settings are needed.

The 3 cases of 2019-nCoV infection at Center Hospital of National Center for Global Health and Medicine

We report 3cases of 2019-nCoV infection that diagnosed in our hospital. It is difficult to make a diagnosis based on clinical symptoms because the initial symptoms in the three cases are minor, such as sore throat, nasal discharge, cough, and mild fever. All have had a history of staying in Wuhan and are thought to have been infected in Wuhan, so it is important to have a history of travel to Wuhan for the past 14 days. However, at present, cases are reported all over China other than Wuhan City, and it is considered that in cases suspected from clinical
symptoms, 2019-nCoV infection should be differentiated outside Wuhan City. The majority of cases are in China.
However, the number of cases in Japan including asymptomatic cases is 20 on February 3, but there is no report of clinical features in Japan, and the number may increase in the future. Here, we report three cases experienced in our hospital, because grasping the general clinical picture may contribute to the treatment of 2019-nCoV infection in the future.

For the treatment of methicillin-susceptible Staphylococcus aureus (MSSA) bacteremia, anti-staphylococcus penicillins such as nafcillin and oxacillin or cefazolin are often used. On the other hand, penicillin-susceptible Staphylococcus aureus (PSSA) is a certain proportion in MSSA, and it is said that PSSA bacteremia can be treated with penicillin G or ampicillin, but the evidence is poor. Therefore, NCGM conducted a five-year retrospective cohort study of PSSA bacteremia, indicating that "penicillins (penicillin G and ampicillin) might be a definitive therapy against PSSA bacteremia under stable conditions" and was accepted by the Journal of Infection and Chemotherapy.

Cefazolin has been the first-choice drug to treat penicillin-susceptible Staphylococcus aureus (PSSA) bacteremia, because oxacillin and nafcillin are not approved in Japan. ELIMINATION OF MALARIA – HALF WAY THERE BUT STILL MUCH TO DO.

■要旨:There has been considerable progress towards elimination of malaria during the past two decades with the estimated number of deaths from malaria falling by over a half, from approximately one million deaths per year in 2000 to an estimated 429,000 (Uncertainty Interval 235-639,000) deaths from malaria in 2016 (WHO World Malaria Report 2017). Clinical cases have fallen by 41% and 17 countries have achieved elimination during the same period. These advances have been achieved without the deployment of any new ‘magic bullet’ but because an increase in funding for malaria control has allowed widespread deployment of the malaria control tools that were available 20 years ago ・・・そして薬剤耐性マラリアの拡散がその対策を危うくしているお話しの続きは会場で!!

■要旨:Invasive meningococcal infection is probably a relatively new disease. The first recorded outbreak occurred in Geneva in 1805. The following year, typical cases of meningococcal meningitis (cerebrospinal meningitis) were seen in New England and epidemics occurred across Europe and North America throughout the nineteenth century. In 1840, the first outbreak in Africa was reported among French troops based in Algiers and during the latter half of the 19th century several outbreaks occurred in Egypt and the Sudan. However, it was not until 1905 that the first major epidemic in West Africa was

■講師紹介:Professor Brian Greenwoodis Manson Professor of Tropical Medicine at London School of Hygiene & Tropical Medicine (LSHTM) where he coordinates the Malaria Capacity Development Consortium (MCDC), which is supporting the post graduate training of African scientists in malaria research in five African universities. He also coordinates the African Meningococcal Carriage Consortium which is investing the impact of a new vaccine against epidemic meningitis in seven countries in the African meningitis belt. Before joining the staff of LSHTM he worked for 30 years in West Africa, first in Nigeria where he helped to establish a new medical school at Ahamdu Bello University, Zaria in northern Nigeria and then for 15 years in The Gambia where he directed the Medical Research Council Laboratories. His main research interests are malaria, pneumonia and meningitis in African children.
