كلكوا بتعرفوا انه الـ Warfarin هو عبارة عن anticoagulant
بس بتعرفو انه الـ Warfarin ممكن يزيد الـ
hypercoagulable state during
the first few days .
يعني هو يسوي thrombosis !! 😅
كيف طيب ؟!
الـ Warfarin راح يشتغل على قسمين ⬇️⬇️
- vitamin K–dependent coagulation factors II (pro-
thrombin), VII, IX, and X
ويشتغل على ↙️↙️
natural anticoagulants ( Proteins C and S )
⚫ الـ Warfarin مراح يمنع الـ factors المصنعه سابقا ( يعني الموجودة بالـ circulation )
وانما راح يمنع تصنيع الجديدة
و المشكله كل هاي الـ factors الموجودة بالـ circulation تختلف بالـ half life
يعني مو كلهن راح ينمنعن بنفس الوقت ,, ⬇️⬇️
Factor ll ➡️ 60 to 100 hours
(prothrombin),
Factor Vll ➡️ 6 to 8 hours
Factor lX ➡️ 20 to 30 hours
factor X ➡️ 24 to 40
Proteins C and S ➡️ 8- 10 and
40 - 60 hours respectively.
- هذا الاختلاف بالـ half life راح يسببلي مشكله
بسبب ملاحظه مهمه والي هي ⬇️
Reductions in the concentration of natural anticoagulants before the clotting factors are depleted can lead to a paradoxical hypercoagulable state.
لهذا السبب ↙️↙️
patients with acute thrombosis should receive a fast-acting anticoagulant (heparin, LMWH, or fondaparinux)
while transitioning to warfarin therapy.
