DNAR(DNR) အကြောင်းတွေ ပြောကြတော့

ကိုယ်တွေ Emergency medicine အနေနဲ့ ကြုံရတဲ့

CPRIC (CPR- Induced Consciousness) လို့ခေါ်တဲ့
conscious cardiac arrest အကြောင်းလေး သတိရလာတယ်။

တနေ့ ကိုယ်တွေ dutyမှာ AMI လူနာတယောက်
pulseless VT တွေ runပီး cardiac arrest ရပါရော။
ထုံးစံ အတိုင်း ACLS လုပ်ကြတာပေါ့။
ထူးဆန်းတာက chest compression လုပ်နေတဲ့သူရဲ့လက်ကို arrest ရတဲ့လူနာရဲ့လက်က လာကိုင်တာပဲ။
chest compression သမားလည်း သူ့လက်ကို လူနာက လာကိုင်တော့
ဟာ လူနာလှုပ်လာပီ။
ROSC (Return of spontaneous circulation)ရပီလားပေါ့။
အဲ့တာနဲ့ compression ကို ရပ်ပီး pulse ကိုင်တော့ pulse က မရသေး။လူနာလက်ကလည်း ပြန်ပီး ကုတင်ဘေးကို ကျသွားရော။ အဲ့တာနဲ့ CPR ပြန်စပေါ့။အဲ့မှာ ထူးဆန်းတာက chest compression ပြန်စပီး သိပ်မကြာဘူး လူနာလက်တွေလှုပ်လာပီး compression လုပ်တဲ့လက်ကို လာကိုင်တာပဲ။ROSC ပြန်ရလားဆိုတော့ မရဘူး။အဲ့တာနဲ့CPR ပြန်စ လူနာက ကိုယ့်လက်ပြန်ကိုင်နဲ့တင် ချာလပတ်လည်နေရော။
ကိုယ်တွေလည်း အဲ့ကျမှ စကြုံဖူးတော့ ဘာကြီးလဲပေါ့ သရဲ ပဲ ခြောက်တာလား။
သရဲခြောက်ရအောင်လည်း မီးတွေထိန်လင်းပီး ဒီလောက်ရှုပ်ရှက်ခတ်နေတဲ့ resuscitation room ထဲ ခြောက်စရာလားပေါ့။
ပြသနာက Shockable rhythm ဆိုတော့ Defib ရိုက်မယ်ဆိုရင် သူ့လက်ကြီးက ကိုယ့်လက်လာကိုင်တာပဲ။ အဲ့တော့ ဘာလုပ်ရမှန်းမသိ ။နောက်တော့ ထုံးစံအတိုင်း arrest မဟုတ်တဲ့ patient တွေမှာ electrical cardioversion လုပ်ရင်သုံးတဲ့ midazolam ထိုးပီး defib ရိုက်ရတယ်။

အဲ့လိုလုပ်နေရင်း နောက်ကျတော့ patientက ROSC ရသွားရော။ ကိုယ်တွေလည်းအဲ့ကျမှ ဟင်းချနိုင်တယ်။

နောက်မှ စာတွေရှာဖတ် ဆရာတွေမေးကြည့်မှ
ဒီလို conditionကို
CPRIC (CPR Induced Consciousness) လို့ခေါ်မှန်း သိတော့တယ်။

အဲ့တော့ CPR IC ဆိုတာ ဘာလဲ
CPR IC ရဲ့ ထူးခြားချက်ကဘာလဲ
CPR IC ကို ဘယ်လို management လုပ်ကြမလဲ

CPR IC ဆိုတာ
one or more of spontaneous eye opening. jaw tone. speech or body movement in pulseless patients undergoing active CPR လို့ ဆိုတာပဲ။
အဲ့တော့ ပုံမှန်ဆို ဘာ movement ဘာ tone မှ မရှိရမဲ့ cardiac arrest patientမှာ CPR လုပ်နေတုံး မျက်လုံးကြီး ပြူးပြတာ စကားတွေ ထပြောတာ (အသံတချို့ထွက်တာနေမှာပေါ့) လက်တွေ ခြေတွေ လှုပ်လာတာပေါ့။
ကိုယ်တွေ တွေ့ဖူးတဲ့ CPRIC 4 cases လောက်မှာတော့ မျက်လုံးပြူးပြတာနဲ့ အသံထွက်တာတော့ မပါဘူးရယ်။ ခြေလှုပ်လက်လှုပ် ရုန်းကန်လှုပ်ရှားတာပဲ တွေ့တာ။

Cause ကို သေချာပြောဖို့ခက်ပေမဲ့
CPR IC ဆိုတာ
high quality CPR ဖြစ်တာရယ်
အဲ့ high quality CPR ကြောင့် cerebral perfusion တက်လာကြောင့်လို့ ယူဆကြတယ်ဆိုပဲ။

Younger male. initial rhythm of VF or pVT. shorter time interval between cardiac arrest and provision of emergency medical care. early high quality CPR with minimal interruption
တွေမှာ ပိုတွေ့ရတယ် ဆိုပဲ။

CPR IC ရဲ့ကောင်းတဲ့အချက်ကတော့
independently associated with ROSC and survival to hospital discharge ပါပဲ။

Management recommendation for CPR IC ကတော့
sedation and physical restraint လို့ပြောတယ်။ (still needs more research)

Sedation မှာ သုံးလို့ကောင်းတာတွေက
Midazolam.နဲ့ Ketamine လို့ပြောတယ်။
Ketamine ကတော့ BPမကျတော့ ပိုပီးသုံးလို့ကောင်းတယ်လို့ ဆိုတယ်။

physical restraint ကတော့ တမျိုးကြီးပဲပေါ့နော် CPR လုပ်တယ်လည်း ဆိုသေး လူနာကြီးကို ချုပ်ပီး CPR လုပ်နေတာဆိုတော့။
physical and psychological risk ရှိတာပေါ့လေ။

အဲ့တော့ CPR လုပ်ရင်း လူနာကမျက်လုံးကြီး ပြူးပြ အသံတွေထွက် လက်တွေ လာကိုင်ရင် ROSC ပြန်ရတာလည်း ဖြစ်နိုင်သလို CPR IC လည်း ဖြစ်နိုင်ကြောင်းလေးပြောပြတာပါ။
ဘာပဲ ဖြစ်ဖြစ် CPR IC ဆိုရင် ကိုယ်လုပ်နေတဲ့ CPR သည် effective and high quality လို့ ဂုဏ်ယူလိုက်ပါလို့။.....
