What drives healthcare utilization in Malawi? 📍🏘️ _ _ _ _ _ _ _ _ _ _ _📍🏨

Findings from the People’s Voice Survey show that 64% of respondents choose public hospitals primarily because they are closer and easier to reach. This highlights how geographic accessibility remains a critical factor in whether people seek care when they need it.

💡These insights provide important evidence for strengthening service delivery and guiding strategic investments in health infrastructure, facility readiness, and community-level access to care.

For the Ministry of Health, the People’s Voice Survey provides citizen-generated data to inform planning and resource allocation across the health system.

Charles Jenya, the Knowledge Management and Learning Lead at the Learning Centre for Quality Management will be speaking in a session on Health Workforce Management and Performance, sharing Malawi's experience in expanding credit-bearing CPD in patient safety through e learning.

Chronic illnesses require consistent care, regular follow-up, and strong connections between health facilities, health workers, and communities. 🏘️🏨

The People’s Voice Survey found that 60% of women reported living with a chronic illness, yet only 21% regularly follow up for care. This highlights an important opportunity to strengthen continuity of care for women across the health system.

Chronic illnesses include conditions such as Hypertension, Type 2 Diabetes, Asthma, and HIV/AIDS, which require ongoing care and regular medical follow-up.

Regular follow-up visits are essential for:
✔ Adjusting treatment plans
✔ Early detection of complications
✔ Preventive counseling and screening

Together, health facilities and front-line health workers can strengthen follow-up care by:
✔ Strengthening patient follow-up systems
✔ Improving appointment reminders and community outreach
✔ Ensuring consistent availability of medicines
✔ Promoting patient education on chronic illness management
✔ Strengthening collaboration with community health workers

Stronger follow-up systems can help ensure women receive the continuous care they need to live healthier lives.

What happens when medicines are unavailable? 💊
What happens when the nearest facility is too far? 🏨

The People’s Voice Survey (PVS) shows that 80% of rural-based respondents had to borrow money or sell personal items to access healthcare in 2024. The unavailability of medicines in public health facilities, as well as long distances to health facilities, were the major reasons respondents resorted to borrowing money or selling personal items.

The survey indicates a clear pattern: when medicines are unavailable and facilities are far, households are forced to absorb the shock.

This demonstrates that healthcare access is deeply interconnected with economic stability. Addressing these barriers requires integrated planning to ensure that PVS findings inform departmental strategies and annual joint work plans across ministries.

What can we do better?
Strengthening supply chains, improving facility readiness, and investing in accessible infrastructure should be reflected in joint annual work plans. Health financing and service delivery reforms must go hand in hand to reduce the economic burden on Malawians.

We don’t build strong institutions alone. We build them together. 🏨
LCQM believes real institutional strengthening happens through partnership and shared expertise. We are grateful to NHS England for their continued support.

They worked closely with us to review and strengthen the Improvement Coaches course curriculum and training materials, ensuring they are practical and aligned with global standards. Beyond this, they’ve built our team’s capacity to confidently deliver high-quality, competency-based training and supported our transition toward accessible, e-learning approaches.

Since January, NHS England has trained us in:
1. Flipped classrooms, where participants engage with core material in advance and use live sessions for practical application.
2. Storytelling with data, translating evidence into clear narratives of progress and impact.
3. Change management, helping teams adopt and sustain new ways of working.
4. Project management, delivering structured, measurable improvements on time.

These capabilities directly strengthen the work of LCQM and contribute to a more resilient, quality-focused health system.

From Training to Implementation: Strengthening Quality Improvement Reporting at Facility Level

Quality Management Directorate (QMD) zonal office, in collaboration with DHOs, are currently conducting facility-based orientations on Quality Improvement reporting across six districts: Kasungu, Ntchisi, Rumphi, Mzimba, Nsanje and Chikwawa. The initiative will reach 60 health facilities and approximately 600 front-line health workers.

Using a cascade model, recently trained Trainers of Trainers (ToTs) are now implementing lessons learned by orienting their colleagues on how to report QI projects through DHIS2. This district-led approach builds sustainable capacity for ongoing QI reporting training within facilities.

QMD zonal teams are leading the activity, providing mentorship, supporting trainers, ensuring user credentials are created and guiding facilities to develop QI action plans.

The activity is led by the Ministry of Health in partnership with UNICEF, PACHI and the Learning Centre for Quality Management.

A Time-Motion Study is underway! The Ministry of Health is piloting integrated health service delivery in Rumphi District and Nkhotakota District as part of the Health Sector Strategic Plan III (HSSP III) reforms.

To strengthen evidence-informed decision-making, Kamuzu University of Health Sciences (KUHeS), with support from the Gates Foundation, is implementing a Time-Motion Study in collaboration with the Learning Centre for Quality Management (LCQM).

The study applies minute-by-minute observation to document health worker activities in real time, capturing location, patient condition and specific tasks. This structured analysis is designed to generate practical evidence to guide policy refinement and optimize integrated service delivery.

The goal is clear: to understand how integration can further enhance efficiency while sustaining quality of care within Malawi’s health system.

The People’s Voice Survey results showed that only 13% of health visits in Malawi, in 2024, were for preventive care.
For quality improvement teams, this is not just a statistic - it is a signal.

It highlights a system opportunity: integrating preventive services into routine workflows can reduce avoidable complications, ease facility congestion, and lower long-term costs.

The Ministry of Health will use the People’s Voice Survey findings to guide action. Through structured Quality Improvement approaches, facilities can embed prevention into triage, clinical pathways and routine service delivery.

Preventive care, including blood pressure screening, cervical cancer screening, immunization, early ANC booking, growth monitoring and routine HIV testing, should not be an add-on. It should be part of standard care processes.

Integrate preventive services into routine workflows.

Keep in mind, Prevention today → Fewer emergencies tomorrow → A stronger health system.

In December, the Ministry of Health officially launched the findings from the People’s Voice Survey (PVS) through a high-level webinar.

The findings capture the voices of Malawians and their perceptions of health service delivery across the country, highlighting lived experiences, what is working well, and where improvements are needed to strengthen our health system.

These insights are critical for health workers and stakeholders committed to advancing health sector outcomes. Over the coming weeks, the Learning Centre for Quality Management will share key PVS findings on our social media platforms. We invite you to follow along and be part of this journey toward improved health service delivery in Malawi.
