We’ve all heard the stories: You prepare for surgery, you fast, you get to the hospital, and then—at the last minute—you are told to go home and come back in three months.

The common myth is that doctors cancel surgeries to force patients into their private clinics for money. But as someone at Queen Elizabeth Central Hospital (QECH), I want to share the reality of what actually happens behind those theater doors.

1. The "Emergency Leapfrog" 🚑
At Queens, we do not have a dedicated theater just for emergencies. If a victim of a road accident comes and needs an emergency operation they will be rushed in, they cannot wait. To save a life right now, we have to stop the "elective" (planned) list.
Your surgery isn't cancelled because we don't care; it’s cancelled because someone else is about to die if they don’t get operated within an hour or 2.

2. Resource constraints: some operations require skills that only one or two surgeons posses in the department. If that surgeon is away, the other more junior surgeons cannot proceed without oversight.

Some operations require admission to the intensive care unit after the operation. QECH only has 7 ICU beds, if the night before your operation an emergency occurred and a patient was admitted on the bed booked for you, we have to cancel.

Anaesthesia!!! Akulu akulu, pain has to be eliminated for a successful operation. We cannot be operating while you scream in pain. If we do not have an anaesthetist (because the numbers are lacking) allocated to our theatre, the list will be canceled.

3.The Booking "Train" Never Stops 🗓️
Think of the surgical list like a train that is always moving.
On Monday, we see patients in the clinic and book them for the next available dates.
On Tuesday, we see more patients and fill the next gaps.
This happens every single day.
