Dr Vijaya Ramasamy is a Consultant Nephrologist & Physician at Island Hospital, Penang. Prior to returning to Malaysia in 2018, he was a Consultant Nephrologist & Physician at Wrexham Maelor Hospital (WMH), UK since 2015. Here, Dr Vijaya also served as a Clinical Lead for acute kidney injury (AKI) services for North Wales and a national AKI steering committee member, as well as being the lead nephrologist for living donor kidney transplantation. Dr Vijaya qualified from University of Dundee, UK with Bachelors of Medicine and Surgery (MBChB) in 2006. After completing his early post-graduate training in internal medicine and obtaining MRCP qualifications, he undertook specialist training in Nephrology and Internal Medicine at University Hospital of Wales, Cardiff and WMH between 2010 and 2015, where 2 of these years was spent in the kidney transplant centre. Dr Vijaya then passed his Specialist Certificate Examination in Renal Medicine in 2013 (SCE Nephrology) and obtained Certificate of Completion of Training (CCT) in Nephrology and Internal Medicine in 2015. In the same year he was entered onto the GMC UK Specialist Register for Nephrology and General Internal Medicine and still remains in the register as a specialist in the UK. He is also registered with the Malaysian National Specialist Register (NSR) for both Nephrology & Internal Medicine. Dr Vijaya’s specialist interests includes kidney transplantation, hypertension, diabetic nephropathy and acute kidney injury (AKI). He is actively involved in education, quality improvement projects and research in these fields, including being site lead for multicentre studies. A lot of his work has been published and presented in various national and international congresses / conferences, winning prizes and grants, including the coveted 2014 European Renal Association Grant. Dr Vijaya was involved in cutting edge HLA incompatible kidney transplantation whilst he was attached to the kidney transplant centre and was even invited to present the centre’s experience in the European Society of Transplantation (ESOT) Congress at Brussels, Belgium in September 2015. Dr Vijaya is also extremely passionate about teaching and training, culminating in him being appointed as the Royal College of Physicians (RCP) College Tutor in August 2016. He is also a very well-known speaker in local and international meetings / conferences, a key opinion leader for many pharmaceutical firms and sits in various national advisory boards. In 2018, Dr Vijaya was elected to the fellowship of The Royal College of Physicians of London (FRCP) for his contribution to education and clinical services, and at that time was one of the youngest members to be elected as a fellow of the prestigious London College of the RCP.

Hyperkalemia (high potassium levels in blood) can be a life threatening complication of kidney disease and may sometimes be an inevitable consequence of some life saving heart medications. In the past, we had to rely on very archaic way to manage this - which often means admitting them to administer this treatment. This of course has huge cost and lifestyle implications. Cardiologists also sometimes shy away from using these life saving heart medications - worrying about these consequences. However, things are about to change. Malaysia is about to witness a revolutionary new treatment called Lokelma, which can facilitate patients to remain on their life saving heart medications and at the same time may even prevent unnecessary admissions. Lokelma is already available in Singapore and we were delighted to welcome Dr Alvin Ng from Mount Elizabeth Hospital, Singapore to share his experiences. I spoke beforehand on the huge issues surrounding the burden of hyperkalemia and the desperate need for newer therapies.

Malaysia had the great pleasure of welcoming Professor Hiddo Lambers Heerspink to Kuala Lumpur last month, where he delivered a keynote address on SGLT2 inhibition for chronic kidney disease (CKD). Professor Heerspink is one of the world's leading authority on SGLT2 inhibitors and is the principal investigator for the landmark DAPA CKD Study, which had revolutionised CKD treatment. It was a real honour to be invited to speak alongside him, where I spoke on how we could translate the findings from clinical trials into clinical practice, for the benefit of our CKD patients

It was a great pleasure to welcome this year's National GP Symposium to Penang. I once again spoke on the urgent need to improve BP control in Malaysia in particular and south-east asia in general, sharing the strategies envisaged by the international societies and the World Health Organization for this objective. I was also a panellist in the interactive diabetes session, incorporating kidney considerations into the lively discussion.

I was in the beautiful city of Kota Kinabalu, Sabah last weekend, addressing Hot Topics In Cardiology, organised under the auspices of National Heart Association Malaysia and Singapore Cardiac Society. Once again I delivered a talk on the role of single pill combination (SPC) in achieving benefits beyond blood pressure lowering in patients with hypertension - which is currently under utilised in this region. It was a real privilege to share the stage with the giants of the field from Malaysia and Singapore.

Today, I delivered a talk to doctors in Kuala Lumpur on non-diabetic chronic kidney disease - an entity which has not received much attention until very recently due to lack of available therapies. That has changed now following the publication of the DAPA CKD trial, we now have more options for these group of patients and this talk was to raise such awareness.

It was a real honour to be invited to be part of Astra Zeneca's advisory board for hyperkalemia in Malaysia. Hyperkalemia is a potentially life threatening condition, most commonly arises due to kidney dysfunction and side effects of certain medications. There is a huge unmet need for treatment in this field and I am glad we have a new therapy in the horizon, Sodium Zirconium Cyclosilicate (SZC).

Spoke on early, intensive glycemic control to prevent diabetic kidney disease and later on challenges of managing diabetes in patients with chronic kidney disease at the P2P Scientific Forum. Great attendance from doctors from all over Malaysia - both GPs and specialists.
