HIV Testing: A Conversation Every Sexually Active Man Needs

Gentlemen, let’s talk about something many men feel uncomfortable discussing β€” but it is extremely important for your health and the health of your partners.

HIV testing is not just for β€œother people.”
It is for every s*xually active man who wants to stay healthy, responsible, and in control of his future.

Why This Matters for Nigerian Men

As a urologist practicing in Nigeria, I often see how stigma and fear around HIV testing stop men from getting tested. Many men avoid testing because they assume they are fine or worry about what others might think.

But the truth is simple: many men living with HIV do not know their status. When someone does not know their status, they are not receiving treatment, they may unknowingly expose partners, and their health can quietly deteriorate.

Testing removes uncertainty. It gives you the power to take action early and protect yourself and others.

How Often Should You Test?

Here is what I usually advise my patients:

If you are s*xually active: test at least once a year.

If you have multiple partners or sometimes have unprotected s*x: test every 3–6 months.

If you had unprotected s*x with someone whose status you do not know: do not delay β€” get tested as soon as possible.

Regular testing should be a normal part of every man’s healthcare routine.

What the Test Actually Tells You

An HIV test today is simple, quick, and reliable. Many clinics offer rapid tests with results the same day.

If your result is HIV-negative: that is great news. Continue protecting yourself and remember that your status reflects your most recent risk exposure.

If your result is HIV-positive: it is important to know that HIV is now a manageable medical condition.

With modern antiretroviral therapy (ART), people living with HIV can live long, healthy, and productive lives. When treatment reduces the virus in the body to an undetectable level, it also means the virus cannot be transmitted to s*xual partners β€” a concept known as β€œUndetectable = Untransmittable (U=U).”

Prevention Is Even Better

If you test negative but have a higher risk of HIV exposure, speak with your doctor about PrEP (Pre-Exposure Prophylaxis).

PrEP is a medication taken by HIV-negative individuals that can reduce the risk of acquiring HIV by up to 86–99% when used correctly. For many s*xually active men, this has become a powerful tool for prevention.

After the age of 35, many men begin to notice changes in their bodies. Weight often starts to collect around the belly, energy levels may drop, and conditions like high blood pressure or rising cholesterol become more common. In Nigeria, these changes are increasingly seen as lifestyles become more sedentary and diets include more processed foods.

These factors often go hand-in-hand with increasing blood sugar levels, which can quietly lead to type 2 diabetes if not addressed early.

The encouraging news is that type 2 diabetes is often preventable, especially when men take action early. Prevention does not require complicated measures β€” just consistent healthy habits such as balanced eating, regular movement, weight control, and routine health checks.

Know Your Risk

You may have a higher risk of developing type 2 diabetes if you have:

A family history of diabetes.

Excess belly fat or noticeable weight gain after age 30–35.

High blood pressure or high cholesterol.

A sedentary lifestyle, such as long hours sitting at work with little physical activity.

In Nigeria, these risks are becoming more common due to urban lifestyles, reduced daily movement, and diets high in refined carbohydrates.

Practical Prevention Habits That Work

The following simple habits can significantly reduce diabetes risk:

Move every day
Regular activity improves how the body uses insulin. Even brisk walking for 30 minutes most days can make a major difference.

Maintain a healthy weight
You do not need dramatic weight loss. Even losing a small amount of weight can significantly reduce diabetes risk.

Eat for stable blood sugar
Focus on foods that release energy slowly and support overall health:

More vegetables, beans, fruits, and whole grains

Lean proteins such as fish, eggs, and legumes

Fewer sugary drinks, pastries, and highly refined carbohydrates

Traditional Nigerian foods like beans, vegetables, okra, and unprocessed grains can be excellent parts of a balanced diet when portions are controlled.

Sleep well and manage stress
Poor sleep and long-term stress can increase cravings and weight gain, which may worsen blood sugar control over time.

Do Not Skip Screening

One of the challenges with diabetes is that many men feel completely healthy while their blood sugar is already rising.

A simple blood sugar test at a clinic or with your doctor can detect early warning signs such as prediabetes, when lifestyle changes are most effective.

If you are over 35, especially if you have belly fat, high blood pressure, or a family history of diabetes, make it a habit to check your blood sugar at least once a year.

If you’re a man over 40 and you wake up at night to urinate, it’s common. But if it happens often, don’t ignore it. Sometimes it’s small and easy to fix. Sometimes it’s an early warning sign.

When it’s normal

If it happens after drinking plenty water, tea, coffee, or alcohol in the evening, that’s understandable. During hot weather, many men drink more fluids and may wake up once at night.

When to pay attention

If you wake up two or more times most nights, and it affects your sleep, energy, or mood, take it seriously. Poor sleep over time affects your health. Night urination is often a symptom, not the main problem.

Common causes in men 40+

Enlarged prostate (BPH)
You may notice weak urine flow, difficulty starting, stopping and starting, or feeling like the bladder is not empty.

High blood sugar / diabetes
When blood sugar is high, the body removes it through urine. This leads to passing more urine, often noticed first at night.

Leg swelling in the evening
If your ankles swell, when you lie down the fluid returns to circulation. Your kidneys filter it, and you produce more urine overnight.

Sleep problems
Light sleep, loud snoring, or sleep apnoea can wake you up more often. Once awake, you feel the urge to urinate.

What you can do this week

Stop drinking fluids 2–3 hours before bed.

Reduce caffeine after afternoon.

Try cutting down alcohol for one week.

Check for ankle swelling in the evening.

Keep a simple 3-day note: sleep time, how many times you wake to urinate, and what you drank after 6pm.

See a doctor urgently if you notice:

Burning pain when urinating, fever, or feeling unwell.

Blood in urine.

New back or side pain with urinary symptoms.

Sudden change in urination pattern.

Night urination disturbing your sleep most nights.

Many men ask me whether they are just stressed or if their testosterone is low. The symptoms can look similar, but there are important differences.

Low testosterone is usually not just tiredness. It often shows up as a steady drop in s*x drive over months, fewer or no morning er****ons, loss of strength, more belly fat, and a flat mood that does not lift. When s*xual changes and body changes happen together and persist, hormones become a real possibility.

Stress feels different. Sleep is poor because the mind will not switch off. Energy goes up and down depending on work pressure or money worries. Sexual performance may improve when you are rested or on holiday. That up-and-down pattern is more typical of stress than true hormonal deficiency.

The honest truth is that you cannot diagnose this by symptoms alone. Both stress and low testosterone can cause fatigue, low mood, poor focus, and reduced interest in s*x. That is why proper evaluation matters. A morning testosterone test, a review of sleep, alcohol, exercise, blood pressure, and weight help us see the full picture.

In Nigeria, hydration can be challenging. Our weather is hot for most of the year, many men work outdoors, and some deliberately β€œdrink less” to avoid frequent urination. The problem is that too little fluid can irritate the bladder and increase the risk of kidney stones β€” especially in our warm climate.

How much water is enough

There isn’t one perfect number for every man because climate and physical activity affect your needs.

A practical starting point used in many health guidelines is about 3.7 litres of total fluids per day for men. This includes water, other drinks, and fluids from food β€” not just plain water.

If you work outside in Lagos, Abuja, Port Harcourt, or anywhere with high heat and humidity, you may need more due to sweating.

A simple rule that works in real life

Use your urine colour as a guide.

Dark yellow urine usually means you are not drinking enough.

Pale straw colour is generally a good sign of proper hydration.

If you are drinking so much late at night that you wake up several times to urinate, shift most of your fluid intake to earlier in the day.

If you urinate frequently, don’t make this mistake

Many men with urinary frequency drastically cut back on water. That often makes things worse.

When urine becomes concentrated, it can irritate the bladder lining and increase urgency and frequency. Instead, aim for steady hydration throughout the day, then gradually reduce intake a few hours before bedtime.

Watch the bladder irritators

Caffeine (coffee, energy drinks, some soft drinks) can irritate the bladder and increase urgency. Reducing caffeine often helps more than cutting water.

Alcohol, especially in the evening, can worsen bladder symptoms for many men.

Hot weather and kidney stones

In hot conditions, you lose more fluid through sweating. Dehydration reduces urine volume and concentrates stone-forming substances, which increases the risk of kidney stones β€” a condition we commonly see in Nigerian hospitals.

If you experience:

Severe side or back pain

Nausea or vomiting

Blood in the urine

Do not ignore it. Seek urgent medical attention.

The goal is simple: maintain steady hydration, protect your kidneys, and support long-term bladder health.
