Overview

Management (Treatments and drugs) for low s***m count include

1. Surgery. Some disease conditions in the male reproductive parts can reduce/ affect s***m production and function. Surgical correction of these disorder can help maximize s***m quality and function. For example, a varicocele (swelling in the sc***um) can often be surgically corrected or an obstructed vas deferens (a vital part of the male reproductive part) repaired. Also, prior vasectomies (surgery to cut the flow of s***m) can be reversed. In cases where no s***m are present in the ej*****te, s***m can often be retrieved directly from the testicles using s***m retrieval techniques.

2. Treating incriminating infections. Antibiotics can cure an infection of the reproductive tract, but, please note this doesn't always restore fertility.

3. Treatments for s*xual in*******se problems. Medication or counseling can help improve fertility in conditions such as erectile dysfunction or premature ej*******on.

4. Hormone treatments and medications. In some instances, your doctor might recommend hormone replacement or medications in cases where infertility (low s***m count) is caused by high or low levels of certain hormones or problems with the way the body uses hormones.

5. And, if all the above mentioned failed, then, “Assisted reproductive technology” (ART) will be the last resort. This treatments involve obtaining s***m through normal ej*******on, surgical extraction or from donor individuals, depending on your specific case and wishes. The s***m are then inserted into the female ge***al tract, or used for in vitro fertilization IVF
And, what if the treatment doesn't work

In rare cases, unfortunately male fertility (low s***m count) can’t be treated, and it's impossible for a man to father a child. If this is the case, you and your partner can consider either using s***m from a donor or adopting a child.

Lifestyle and home remedies

There are steps you can take at home to increase your chances of getting your partner pregnant, if you have low s***m count, these include:

1. Increasing the frequency of s*x. Having s*xual in*******se every day or every other day beginning at least four days before ovulation increases your chances of getting your partner pregnant, even with low s***m count. KEEP TRYING. DONT GIVE UP

2. Having s*x when fertilization is possible. A woman is likely to become pregnant during ovulation, which normally occurs in the middle of the menstrual cycle, between periods. This will ensure that s***m, which can live several days, are present when conception is possible. ( Targeting the fertile window)

3. Avoiding lubricants. Some products such as Astroglide or K-Y jelly, lotions, and saliva might impair s***m movement and function and further worsen the low s***m count. Please, ask your doctor about s***m-safe lubricants.

● GeneralPrevention

To protect your fertility, avoid known factors that can affect s***m count and quality.

For example:

1. Don't smoke.
2. Limit or abstain from alcohol.
3. Steer clear of illicit drugs.
4. Talk to your doctor about medications that can affect s***m count.
5. Maintain a healthy weight.
6. Avoid heat.
7. Manage stress.
8. Avoid exposure to pesticides, heavy metals and other toxins.

When S*x Becomes Painful, What You Should Do And Know.

In*******se pain, or dyspareunia, can cause problems in a couple's s*xual relationship. In addition to the physically painful s*x, there is also the possibility of negative emotional effects. So the problem should be addressed as soon as it arises.
● What Causes Painful S*x in Women?
In many cases, a woman can experience painful s*x if there is not sufficient va**nal lubrication. When this occurs, the pain can be resolved if the female becomes more relaxed, if the amount of foreplay is increased, or if the couple uses a s*xual lubricant.

In some cases, a woman can experience painful s*x if one of the following conditions is present:

1 • Vaginismus. This is a common condition. It involves an involuntary spasm in the va**nal muscles, sometimes caused by fear of being hurt.

2 • Vaginal infections. These conditions are common and include yeast infections.

3 • Problems with the cervix (opening to the uterus). In this case, the p***s can reach the cervix at maximum pe*******on. So problems with the cervix (such as infections) can cause pain during deep pe*******on.

13 • Vulvodynia. This refers to chronic pain that affects a woman's external s*xual organs -- collectively called the v***a -- including the l***a, cl****is, and va**nal opening.

It may occur in just one spot, or affect different areas from one time to the next. Doctors don't know what causes it, and there is no known cure. But self-care combined with medical treatments can help bring relief.



● How Can Painful S*x In Women Be Treated?

~ Some treatments for painful s*x in women do not require medical treatment. For example, painful s*x after pregnancy can be addressed by waiting at least six weeks after childbirth before having in*******se. Make sure to practice gentleness and patience. In cases in which there is va**nal dryness or a lack of lubrication, try water-based lubricants.

~ Some treatments for female s*xual pain do require a doctor's care. If va**nal dryness is due to menopause, ask a health care professional about estrogen creams or other prescription medications. Other causes of painful in*******se may also require prescription drugs.

~ For cases of s*xual pain in which there is no underlying medical cause, s*xual therapy might be helpful. Some individuals may need to resolve issues such as guilt, inner conflicts regarding s*x, or feelings regarding past abuse.

Hepatitis B Guidelines for Pregnant Women

What is hepatitis B?
Hepatitis B is the most common serious liver infection in the world, caused by the hepatitis B virus (HBV) which are known to attack the cells of the liver cells therefore cause liver failure, cirrhosis (scarring) or cancer of the liver later in life.

The virus is transmitted through contact with infected blood and bodily fluids.

If I am pregnant, should I be tested for hepatitis B? Yes!
Pregnant women who are infected with hepatitis B could infect their baby during pregnancy or delivery. Up to 90% of these babies will become chronically infected with hepatitis B at birth if there is no prevention.

ALL pregnant women should be checked and tested for hepatitis B to prevent infection.

What if I test positive for hepatitis B while I am pregnant?
If you are pregnant woman and you have a positive test for hepatitis B infection, you would need to see a liver specialist for further evaluation. However most of the women would not have any pregnancy complications as a result of HBV infection, it is always better to get checked up.

How can I protect my newborn from hepatitis B?
If you are pregnant and you test positive for hepatitis B, your newborn child must be given two shots in the delivery room - the first dose of hepatitis B vaccine and one dose of hepatitis B immune globulin (HBIG). If these two medications are given correctly within the first 12 hours of life, it gives the newborn a better chance , actually a 95% chance of being protected against a lifelong hepatitis B infection. However, the baby would need additional doses of hepatitis B vaccine at one and six months of age to provide complete protection.

If your baby does not receive these drugs at the appropriate times, then there is a greater than 90% possibility that your baby could become chronically infected. **There is no second chance!**

It is vitally important that all newborns be vaccinated at birth against hepatitis B!

Can I breastfeed my baby if I am infected with hepatitis B?
The WHO and the CDC has declared it safe for an infected woman to breastfeed her child. All women with hepatitis B are encouraged to breastfeed their babies since the benefits of breastfeeding outweigh the potential risk of transmitting the virus through breast milk. In addition, since all newborns should receive the hepatitis B vaccine at birth, the risk of transmission is therefore even reduced further.

How will I know if I am infected with hepatitis B?
***All pregnant women should be tested for hepatitis B.***

The 3-part hepatitis B blood panel includes the following:

1. Hepatitis B Surface Antigen (HBsAg): The “surface antigen” is part of the hepatitis B virus that is found in the blood of someone who is infected. If this test is positive, then the hepatitis B virus is present.

2. Hepatitis B Surface Antibody (HBsAb or anti-HBs): The “surface antibody“ is formed in response to the hepatitis B virus. Your body can make this antibody if you have been vaccinated, or if you have recovered from a hepatitis B infection. If this test is positive, then your immune system has successfully developed a protective antibody against the hepatitis B virus. This will provide long-term protection against future hepatitis B infection.

3. Hepatitis B Core Antibody (HBcAb or anti-HBc): This antibody does not provide any protection or immunity against the hepatitis B virus. A positive test indicates that a person may have been exposed to the hepatitis B virus. This test is often used by blood banks to screen blood donations. However, all three test results are needed to make a diagnosis.

How do I protect my child if another family member is infected with hepatitis B?
Babies and children can be exposed to HBV from an infected dad, sibling, or other family member living in the same household. This can occur through contact with infected blood and bodily fluids. Vaccination is the best prevention against spreading the hepatitis B virus!

How can I prevent getting hepatitis B if someone in my household is infected?the recommendation is that anyone living in a household with an infected family member should be
vaccinated.

Until your vaccine series is complete, it is important to avoid sharing any sharp instruments such as razors, toothbrushes, or earrings, etc. since small amounts of blood can be exchanged through these items. Also, infected individuals should be careful to keep all cuts properly covered. Blood spills should be cleaned with gloves and a 10% bleach/water solution.

Hepatitis B is not transmitted casually and it cannot be spread through sneezing, coughing, hugging, or eating food prepared by someone who is infected with Hepatitis B.

Myths and Fictions

■ Majority of times , women freak, fret and fear at the slightest feel of pain in their breasts. The GOOD NEWS, generally, breast pain is not a sign of breast cancer, so breathe easy and relax.

■ It is important to know what is normal in your breasts ,so you can know when abnormality sets in. Women are encouraged to be breast friendly. Make your breasts your best friends.

■ You are stuck with the duo for life, so the earlier you familiarize and become bosom friends the better for your overall breast health.

Facts

♤Should breast pain cause me to be concerned about breast cancer?

♤Fact is, although many women with pain in one or both breasts may be concerned that it is breast cancer, it's most times not. The major cause of breast pain is mastitis, (breast inflammatory process from infection or reaction or even hormonal influence )

♧ Majority of women can have breast tenderness( pain on touch ) and pain, also called mastalgia often.

♧ In these women , the pain may come and go with monthly periods (cyclic) or may not follow any pattern (noncyclic).

♧ Cyclic pain is the most common type of breast pain. It may be caused by the normal monthly changes in hormones.

♧ Note ,this pain usually occurs in both breasts. It is generally described as a heaviness or soreness that radiates to the armpit and arm.

♧ The pain is usually most severe before a menstrual period and is often relieved when a period ends.

♧ Cyclic breast pain occurs more often in younger women.

♧ Most cyclic pain goes away without treatment and usually disappears at menopause.

♧ On the other hand Noncyclic pain is most common in women 30 to 50 years of age.

♧ It may occur in only one breast. It is often described as a sharp, burning pain that occurs in one area of a breast.

♧ Occasionally, noncyclic pain may be caused by a fibroadenoma or a cyst.(Non cancerous growths).

♧ If the cause of noncyclic pain can be found, treating the cause may relieve the pain.

♧ Breast pain can get worse with changes in your hormone levels or changes in the medicines you are taking.

♧ Stress can also affect breast pain. You are more likely to have breast pain before menopause than after menopause.

What Causes Breast Pain?

♤There are a number of harmless causes for breast pain and tenderness that may primarily be related to changes in hormone levels.

These can include:

1. Puberty in girls and sometimes for boys, too

2. Menstruation and premenstrual syndrome (PMS)

3.Pregnancy - more often during the first trimester

4.Days following childbirth as milk comes in

5.Breastfeeding Mastitis, which is caused by a milk duct that is not properly draining and becomes infected, should be treated.

6. Fyi, this has no correlation with cancer, but it can become a serious infection if left untreated.

7.Menopause

♤Having said that, breast pain can be a sign of an ongoing issues with the body. If the pain is persistent, please go see your doctor.

♤ Fyi, in medical practice, we never say never, so yes BREAST PAIN can also be a sign or symptom of cancer, though not common.

Swollen Glands in Kids

So, what is a gland?

Also called Lymph glands (or lymph nodes) are very important part of the body’s defense, more of soldiers against infection and illness and any abnormality. These glands normally contain special groups of cells, called lymphocytes, which act as wall to infection.

The lymphocytes in turn produce substances called antibodies that destroy or immobilize infecting cells or poisons. So, the antibodies are like the foot soldiers that protect our body from invaders! When lymph glands become enlarged or swollen, it usually means that the lymphocytes have increased in number due to an infection or other illness and that they are being called into action to produce extra antibodies to defend our body.

Swollen glands are fairly common among kids for the simple fact, they are easily exposed to various infections and harmful substances.

How will you know if your child’s gland is swollen?

Normally If your child has swollen glands, it will be visible to the naked eye and you’ll be able to feel them too. They also may be tender (painful when touched. Oftentimes, if you look near the gland, you can find the infection or injury that has caused it to swell.
For example, a sore throat often will cause glands in the neck to swell, or an infection on the arm will produce swollen glands under the arm. Swollen glands are common in kids! Don’t go fretting or freaking out when you see one!

However, sometimes the illness th may be a generalized one, such as those caused by a virus, in which case many glands might be slightly swollen. In general, because children have more viral infections than adults, lymph nodes, particularly in the neck, are more likely to be enlarged.
PLEASE NOTE, Swollen glands at the base of the neck and just above the collarbone may be an infection or even a tumor within the chest, and should be examined by a physician as soon as possible.

Treatment & Prevention

Majority of cases of swollen glands are not serious and will usually disappears after the illness that caused it is gone. Normally the glands gradually return to normal over a period of weeks.
PLEASE NOTE, you should call the pediatrician if your child shows any of the following:

1. Lymph glands swollen and tender for more than five days
2. Fever higher than 101 degrees Fahrenheit (38.3 degrees Celsius)
3. Glands that appear to be swollen throughout the body
4. Tiredness, lethargy, or loss of appetite
5. Glands that enlarge rapidly, or the skin overlying them turning red or purple

• As with any infection, if your child has a fever or is in pain, you can give her acetaminophen (Paracetamol) in the appropriate dosage for her weight and age until you can see the pediatrician.
• Please remember to keep a diary of the time frame of the swollen glands. When you call, your doctor probably will ask you some questions to try to determine the cause of the swelling, so it will help if you do a little investigating beforehand. For instance, if the swollen glands are in the jaw or neck area, check if your child’s teeth are tender or her gums are inflamed, and ask her if there is any soreness in her mouth or throat.
• Remember to mention to your doctor any exposure your child has had to animals (especially cats) or wooded areas. Also check for any recent animal scratches, tick bites, or insect bites or stings that may have become infected.
• The treatment for swollen glands will depend on the cause. If there’s a specific bacterial infection in nearby skin or tissue, antibiotics will clear it, allowing the glands gradually to return to their normal size.

• However, if the gland itself has an infection, it may require not only antibiotics but also warm compresses to localize the infection, followed by surgical drainage. If this is done, the material obtained from the wound will be cultured to determine the exact cause of the infection. Doing this will help the doctor choose the most appropriate antibiotic.

• In the event that your pediatrician cannot find the cause of the swelling, or if the swollen glands don’t improve after antibiotic treatment, further tests will be needed. Special tests can confirm this diagnosis. In cases where the cause of a swollen gland is unclear, the pediatrician also may want to do a tuberculosis skin test.
• If the cause of prolonged swelling of lymph nodes cannot be found in any other way, it may be necessary to perform a biopsy (remove a piece of tissue from the gland) and examine it under a microscope. In rare cases this may reveal a tumor or fungus infection, which would require special treatment.
Prevention
Of all the causes of swollen glands, the only swollen glands that are preventable are those that are caused by bacterial infections in the surrounding tissue. In cases of suspected infection, you can avoid involving the lymph nodes by properly cleaning all wounds and receiving early antibiotic treatment.

ANC, What, When and Why in Pregnancy

What is ANC

This is a SPECIAL care given to all confirmed pregnant women. It is the routine health check and control of presumed healthy pregnant women without symptoms (screening), with aim of diagnosing any ongoing diseases or complications that might arise from the ongoing pregnancy. During ANC, pregnant women with/ without symptoms are provided information about lifestyle, pregnancy and delivery.

When

When should ANC start? At what month is the right time to start antenatal?

• This can vary depending on your location, but globally, it is advised, you start as early as possible. Matter of fact, the earlier you start attending antenatal check-ups the better. The World Health Organization recommends that pregnant women should commence ante natal visit at the second missed period.
• So, I will personally say, as soon as you confirm you are clinically pregnant with ultrasound, start. It is better to start early, than to start late!
• The globally accepted schedule is as follows.:
• Start as soon as possible and then attend antenatal check-ups once a month until 28 weeks.
• After, 28 weeks, attend twice a month until you are 36 weeks pregnant.
• After 36 weeks, and during the last 4 weeks of your pregnancy you should go every week.
FYI, this can vary depending on your location and hospital. Women with high risk pregnancy will start EARLY!
• In this WHO recommends, a total of four visits which are made for uncomplicated pregnancies. This is however flexible, it can be more or less depending on the status/ health of the pregnancy. The arrangement is usually flexible.

Why

So, why ANC?

During your first visit, which is called booking visit, you will be subjected to series of investigations, this will generally vary from one region of the world to another based upon the most prevalent health conditions that constitute risk to the pregnant women and the baby.

Recommendation

Investigations During ANC

These will include:

1. Urine test for sugar and proteins.

2. Blood test for blood group, genotype, HIV screen, screen for syphilis, and screen for hepatitis B.

3. An obstetric ultrasound scan done to confirm the wellbeing of the baby. The early ultrasound scan apart from confirmation of the pregnancy is very useful for accurate dating of the pregnancy. Late scan is not, as accurate.

Vaccines

Vaccines Needed in Pregnancy, Any recommended number?

This is once again dependent on your location/ region of the world. There are no fixed numbers. They are given on need basis.
1. Teta**s toxoid immunization is also given to the pregnant women
2. You should get the Tdap vaccine (to prevent, specifically whooping cough/ pertussis)
3. Flu vaccine
4. Hepatitis B
5. Some women may need other vaccines before, during, or after they become pregnant. For example, if you have a history of chronic liver disease, your doctor may recommend the hepatitis A vaccine.
6. If you work in a lab, or if you are traveling to a country where you may be exposed to meningococcal disease, your doctor may recommend the meningococcal vaccine.
