According to CJASN, when it comes to Kidney Health, the evidence speaks for itself. Expanded access to adult genetic testing is critical, especially those with family history or CKD of unknown origin, and emphasizes the clinical utility in diagnosis, treatment decisions, and family care.

🔍 Key Findings
Diagnostic Yield:
Genetic testing confirmed a diagnosis in 40% of adults with chronic kidney disease (CKD).
The yield was highest in cystic kidney disease (62%) and lowest in vascular kidney disease (13%).
Reclassification of Diagnosis:
17% of patients with a positive genetic result had their CKD diagnosis reclassified into a different disease subtype.
Subtypes with Highest Genetic Diagnosis Yield:
Cystic kidney disease – 62%
Ciliopathies – 62%
Tubulopathies – 49%
Tubulointerstitial kidney disease – 32%
Glomerulopathies – 27%
CKD of Unknown Etiology (CKDu):
Made up 13% of all participants.
Had a 20% diagnostic yield with genetic testing.
🧬 Genetic Risk Factors & Influencers
Family History: Strongest predictor of a positive genetic test result.
Extra-kidney features: Associated with a higher likelihood of a genetic diagnosis (though not statistically significant).
Age: No significant association between age at testing and likelihood of positive results — challenging age-based eligibility criteria.

🧪 Clinical Impact
Cascade testing in family members occurred in 49% of genetically diagnosed cases.
Treatment changes were made in 18% of diagnosed cases.
Other impacts included:
Screening for extra-kidney features (11%)
Family planning (1%)
Clarification of inheritance (7.8%)

🔬 Examples of Commonly Involved Genes
PKD1, PKD2 – CKD
COL4A3, COL4A4, COL4A5 – Alport syndrome
MUC1, UMOD – Autosomal dominant tubulointerstitial kidney disease (ADTKD)
APOL1 G1/G2 alleles – Common in people of African ancestry

🧭 Implications for Practice
Phenotype-driven testing is useful where the clinical suspicion is high (e.g., cystic kidney disease).
Comprehensive genetic testing (panel/exome/genome) is important for cases with unknown etiology or unclear phenotype.
Testing should not be restricted by age, especially since adult-onset monogenic kidney diseases are common / often missed.
