Grassroots or Top-Down: Which Strategy Best Improves Exclusive Breastfeeding?

In response to a national directive to increase exclusive breastfeeding (EBF) rates within three years, States Djk and Fyj in Country NAJ adopted different strategies.

State Djk focused on grassroots healthcare by training local health workers, village leaders’ wives, husbands and women leaders instead of replacing traditional birth attendants (TBAs). They built trust through appreciation, offered breastfeeding education, provided scholarships, equipped modern facilities, and ensured regular monitoring. This inclusive approach led to a 20% increase in EBF rates.

State Fyj, however, employed new health workers and launched an aggressive advocacy campaign, discouraging mothers from listening to TBAs or family members. This approach triggered resistance from TBAs and community leaders, who defended cultural norms. As a result, Fyj saw only a 2% increase in EBF over the same period.

Kelechi’s Choice: Breastfeeding for Family and National Progress

Kelechi a mother of three from Igbobi, lived modestly with her husband, Felix, a mechanic. After giving birth to baby Amen, she chose to exclusively breastfeed, saving over ₦40,000 monthly on formula and avoiding frequent hospital visits.

Unlike her older, formula-fed child, Amen stayed healthy. The family used their savings to support their children’s education and grow Kelechi’s tailoring business. With improved health and financial stability, Kelechi became more productive and began training other women in her community.

By reducing healthcare costs, improving child health, and empowering mothers like Kelechi to work and earn, breastfeeding not only strengthens families but also contributes to national productivity.

Breastfeeding Interrupted: A Mother’s Journey Through Tradition and Malnutrition

Kuju, born into the pottery-making Ade family, inherited her father's trade and lived in the same village. At 18, she married into the Agabi family, known for basket-knitting. Her first childbirth was assisted by a traditional birth attendant and accompanied by the use of herbs and breastfeeding, which she valued for its affordability and health benefits.

However, her plan to breastfeed long-term changed when she became pregnant again just three months later. Advised by her husband, mother-in-law, and the birth attendant to stop breastfeeding, she introduced the infant to the family diet with minimal attention, and the child often fell ill.

After her second child was born, Kuju gave full attention to the newborn while the first child stayed with the grandmother. Becoming pregnant again when the second child was just five months old, she repeated the cycle. By age two, the second child showed signs of severe acute malnutrition (SAM), including delayed walking and frequent crying.
