Acute Pericoronitis (Dental Emergency)
A 49-year-old patient presented with severe pain from an impacted lower wisdom tooth.
Pain was radiating to the face and ear on the same side.
There was trismus (difficulty opening the mouth), jaw drop, and noticeable warmth.
In the mouth:
▪️ Swelling and redness
▪️ Sloughing and tenderness
▪️ Inflamed gum tissue over the lower third molar
Management:
✔️ Inferior alveolar nerve block using the Akinosi technique
✔️ Debridement of inflamed tissue
✔️ Irrigation with diluted hydrogen peroxide and normal saline
✔️ Prescribed antibiotics, analgesics, and warm saline mouth bath
Next step:
Patient will return after pain and inflammation reduce for possible extraction or operculectomy.
👉 Take-home message:
Painful wisdom teeth with swelling and trismus = don’t delay dental care
