Friends and family, I’m thrilled to share that I’ve been working on a new venture as a Speech-Language Pathologist focused on helping parents with young children experiencing language delays. 70% of the time, SNF residents who have a swallowing impairment are put on a modified diet…including thickened liquids, UNNECESSARILY.

How does this happen? I’m going to talk straight up!

➡️With SLPs sticking to a “Bedside Guess” and moving on
➡️Being afraid to ask for an instrumental objective exam order from the doc
➡️Over-scheduled therapists who need to be “85% productive”
➡️Therapists who are unclear on who needs a proper objective assessment
➡️Quick Fixes because they cough on regular liquids & not on nectar
➡️The patient has dementia and is willing to drink it with no issues
➡️It’s easier!

💰Thickened liquids costs facilities $2000-$7000 PER YEAR PER PATIENT! 😳

📋Give me your list of diets, and we will re-write this story in your building (not to mention, change the quality of many lives AND put lots of $$$ to better use!)

Why is it when a patient is admitted from the hospital with a report stating, “FAILED SWALLOW TEST” everyone begins to nervously bite their nails when the SNF SLP initiates feeding them as part of their assessment?

But wait! Why are we saying FAIL!?

The multiple requirements to swallow food or liquid of any consistency can yield many details that can convey a functional swallow if certain precautions are taken AND…
Remember, the function of the swallow in the hospital can look very different!

My guy today? He “failed” the last evaluation in the hospital just a few days ago. Today he ate and drank 100%! SAFELY!

Pt gets admitted to rehab facility on hospice due to lack of responding due to pneumonia & respiratory failure

Man hasn’t eaten in a week 😳

SLP comes in to “screen” him

✅Mouthbreathing, non- responsive, mouth looks BAD, oxygen in place, breathing with struggle

✅Look inside further with flashlight 😱

📋Back of throat is completely blocked with a large “growth” that extends from the soft palate…is it a loose denture plate?🤔

NO! It’s bacteria! Let’s get to work!!

🧤🪥💦😷

Next day? 🛑 NO MORE HOSPICE!

WITHIN 2 DAYS:

FEES completed, eating 3 meals a day,
sharing his life story, smiling, and THRIVING!

‼️We should not be automatically thickening liquids without instrumentation!

➡️Common practice is for SLP’s to thicken a patient’s liquids because they ARE NOT coughing with thickened liquids and they ARE coughing with thin.

🛑 STUDIES HAVE FOUND THE PATIENTS MAY BE MORE LIKELY TO SILENTLY ASPIRATE THICKENED LIQUIDS, THAN THIN LIQUIDS (Miles et. al 2018).

🫁 There is significant pulmonary harm when thickening agents are aspirated!

🌟Our job is to do no harm‼️

📖Based on current evidence-based research, the use of thickened liquids in the absence of an instrumental swallowing evaluation is contraindicated (in most cases)!

Homework: take inventory of how many of your nursing home residents are drinking thickened liquids without instrumentation.
