TRMD Clinic Patient Guide - For Your Reference

Dear Patients,

Here’s a handy guide to make your visit and communication with us smooth and convenient.

1. Clinic Hours
* Consultations are available from 11:00 AM to 6:00 PM, MONDAY to SATURDAY.
* Messages for clarifications may be sent from 10:00 AM to 7:00 PM, MONDAY to SATURDAY.
* The clinic is CLOSED on SUNDAYS.
* Messages received outside these hours will be attended to on the next clinic day.
* For urgent concerns outside clinic hours, please proceed to the nearest emergency facility.

2. Appointments
* We encourage booking appointments ahead of time.
* Walk-ins may be accepted depending on availability.
* If you will be late or need to reschedule, kindly inform us so we can adjust accordingly.

3. Payments
* Consultation and procedure fees are settled on the same day.
* Modes of payment accepted: Cash, GCash, Bank Transfer.

4. Senior Citizen & PWD Discounts
* We honor Senior Citizen and PWD discounts for all medical consultations and medical services (excluding cosmetic services).

5. Medical Certificates / Fit-to-Work Documents
* Medical certificates and fit-to-work documents are formal medical records that need careful assessment and proper documentation. Since these require additional time to prepare, they have a separate fee and can only be issued after a consultation and when medically appropriate.

6. Document Re-Issuance
* Requests made after the consultation day for re-issuance of lost or misplaced documents
(e.g., prescriptions, certificates, or other clinic paperwork) will have a ₱100 administrative fee.
* This covers:
- Review of your chart
- Re-preparation and verification of details
- Secure processing and sending of documents

7. Follow-Ups
* Follow-up visits are considered separate consultations unless the doctor specifies otherwise.

8. Procedures
* For your safety, please inform us of any allergies, medications, or medical conditions before any procedure.

9. Confidentiality
* All patient information, records, and images are handled with strict confidentiality according to medical privacy standards.
