Hydatidiform mole, or molar pregnancy, "kiyawa" in Tagalog, "saur nga sikog" in Ilocano results from abnormal fertilization of the oocyte (egg). It results in an abnormal fetus. The placenta grows normally with little or no growth of the fetal tissue. The placental tissue forms a mass in the uterus.
Its worldwide incidence is 1-2 per 1,000 pregnancies.
Chance of mole formation is higher in older women. A history of mole in earlier years is also a risk factor.
This condition can turn into a malignancy, hence the need for beta hCG (Tumor marker) monitoring.

2months history of vaginal spotting. Consult done to a private MD wherein ultrasound done revealed invasive mole. However was lost to follow...1 month after patient came back due to vaginal bleeding hence work ups done and started chemotherapy.

- though the patient was categorized as a low risk case but then she developed chemoresistance from the single agent chemotherapeutic drugs...
- not all case are the same...
-treatment is individualized
- trophocases are challenging.. It takes a lot of brain cells to function.. 😜😊

✅additional courses of chemotherapy given after normal values of bHG levels (0-5 mIU/ml) are achieved
✅the number of consolidation courses will depend on the prognostic score of the patients
✅ 2 clean -ups are given for LOW RISK
✅3 clean - upsfor HIGH RISK
IMPORTANCE: 👍🏼this will take care of the tumor cell population (10 ^5) undetected by the hCG immunoassay

scenario 5:
45 yo g3p2 (2012) s/p tahbso mole in situ. beta hcg monitoring done however serum beta hcg dropped was minimal despite hysterectomy was done. the criteria to be diagnosed as gestational trophoblastic neoplasia has not yet meet however patient complained of cough and back pains. hence metastatic work up done. on chest xray there were 13 nodular masses noted. plan: EMACO chemotherapy
