This medical diagnostic principle, suggests that more common diagnoses be prioritized before searching for less probable, rare diseases. But in every rule there is an exception:

🔬 Pathologic Fracture Diagnosed After Traumatic
Long Bone Fracture

⚖️ Case>

55/F drove a solo motorcycle, run through an
uneven road surface, lost control and crashed.
She sustained a closed, complete femur fracture
on the left. Past medical history of completed
radiotherapy for Breast CA a year prior.
Currently under Anastrazole adjuvant therapy.

⚖️ Consideration>

- on initial visual inspection of X-rays, outright
traumatic fracture is apparent
- patient had no prodromal pain on affected
extremity
- precedent high-energy mechanism of injury
- excluding her past medical history, all of the
above mentioned point out to non-pathologic
nature of fracture

🛠️ Intervention>

- on careful visual examination, a perceptible
lytic lesion can be recognized
- further work-up, imaging,and open biopsy
confirmed intuition of pathologic fracture
from metastatic carcinoma
- revision with combined nail - plate - cement
construct for palliative-intent fixation after
initial retrograde nail fixation to afford stiffer
construct. Definitive fixation was done 2 days
after agiogram and embolization of tumor
feeding arteries.

Anterior Subcutaneous Pelvic Internal Fixator (INFIX)
involves placing pedicle/iliac screws into the pelvic bones, which are then connected by a rod placed under the skin.

Indications:
⚒️ unstable anterior pelvic ring
⚒️ elderly patients with fragility fractures
⚒️ obese patients
⚒️ enhanced stability after posterior fixation
⚒️ contraindication for open surgery
⚒️ increased nursing care needs

Case:
♿️ 29/M with complex anterior ring fracture
and bilateral subtrochanteric fracture
sustained from motorcycle accident.
INFIX utilized for pelvic ring injury,
Long Proximal Femural Nail (PFN) fixation
for bilateral subtrochanteric femur fractures

Outcome:
🏥 No lateral femural cutaneous nerve injury,
INFIX removed after 3 months, patient
allowed to mobilize using wheelchair immediately
after surgery

Cervical pedicle screws provide a sturdy and biomechanically sound fixation in the cervical spine which can be utilized in certain conditions complicated by osteoporosis and others such as traumatic injuries, metastatic disease, and revision surgery. In emergency situations accompanied by neurologic deficits, there are always practical limitations that cannot be performed in a state where computer assisted navigation is possible in all surgical environments.

This can be demonstrated in the following case where we utilized our own freehand technique in inserting cervical pedicle screws. The soundness of the short segment construct can be gleamed from the follow -up.

When presented with a case, treatment options vary from simple to complex and everything in between. Shared decision-making with the patient, relatives, and the surgeon should be arrived at to reach the desired outcome annexed with compromises.

Amputation would allow for a quicker recovery, though with significant lifestyle changes. Complex reconstruction would be a long, arduous process - years, potentially, to regain full function. Many surgeries. There’s no guarantee.

When the die has been cast, and no amount of grit and commitment has been spared, faith in God and the belief that good outcome will be favored are all that would be left to cling to and get by.

Indeed battles are worth every single scar. The long nights, the challenging surgeries, the endless encouragement - it was all worth it. Every whimper of pain, every grunt of effort, every clink of a healed bone, has led to this. The patient standing tall, his dreams and aspirations marching towards fruition.
