Majority of pe**le cancer is squamous cell carcinoma, occurs most exclusively in an uncircumcised men, commonly between the ages of 50 and 70 years . Factors associated with intact or redundant prepuce are sm**ma and phimosis. Smegma-induced chronic inflammation may predispose to the development of phimosis as well as SCC. Phimosis, the inability to retract the fo****in over the g***s, is associated with an increased risk of infection as well as dysplastic changes of the preputial sac. Uncircumcised men have been found to be at an increased risk of HPV infection.

Clinically, it may present as a small area of induration and erythema or a large ulcerating and infiltrative lesion. As it progresses, bleeding, itching, discharge, foul odor and pain may be observed.

A case of 16 y/o, M, seen at first because of incarcerated suprapubic foley catheter. 7 years prior, he underwent exploratory laparatomy for abdominal trauma 2 to vehicular accident. Suprapubic cystostomy catheter was placed. Several months later, perineal urethroplasty was performed. 4 weeks post-op, urethral catheter was removed, however urinary retention was experienced several days later. Failed attempted urethral catherization prompted reinsertion of suprapubic cystostomy catheter. Patient was lost to follow-up for 7 years. He replaces his catheter every 4 weeks. 3 months prior, he failed to change his catheter resulting to significant urinary infection and moderate to severe pain. Suprapubic catheter cannot be withdrawn. After several work-ups, perineal anastomosis redo-urethroplasty was eventually performed.

Kidney and Ureteral Stone Treament

◇ American Urological Association (AUA) Guidelines and Best Practice Statements:

■ For KIDNEY stone:

▪︎Symptomatic w/ non-lower pole stone 2cm 👉 PCNL as first line therapy

▪︎ Asymptomatic non-obstructing caliceal stone 👉 may offer active surveillance

▪︎Symptomatic lower pole stone 1cm 👉 PCNL has a high stone-free rate but with greater morbidity. Option: ESWL

■ For URETERAL Stone:

▪︎ Uncomplicated distal ureteral stone < 1cm 👉 observation w/ or w/o MET (medical expulsion treatment) × 4-6 weeks, and if unsuccessful 👉 definitive treatment.

▪︎ Middle or distal ureteral stone that require intervention 👉 URS is the first line therapy. Option 👉 ESWL

▪︎ ESWL has the least morbidity and lowest complication rate. URS has a greater stone-free rate in a single procedure.

▪︎ DJ stenting is not routinely performed in patients undergoing SWL.
