Early full nutrition in ICU patients may suppress beneficial fasting responses and cause harm, including impaired autophagy and hyperglycaemia. Evidence supports withholding full nutritional support during the acute phase. Enteral feeding should be introduced gradually after stabilisation, with parenteral nutrition delayed if needed. Glucose control should avoid severe hyperglycaemia, hypoglycaemia and large fluctuations.
