For a healthy and safe DLSMHSI community, let us beat COVID-19 by following the minimum Health and Safety Protocol.

Good day.

The Student Health and Safety Department will be participating in the Year-End Planning of The Student Affairs on July 13-14, 2022. In these dates, VITAMIN:H.S.I will not able to respond to messages. However, those who are scheduled for follow up and those who completed their isolation periods will be issued with medical certificate/medical clearance as scheduled.

Incoming patients on these dates who have COVID-19 related symptoms are advised to immediately start isolation for 7 days from onset of symptoms and refrain from attending face-to-face academic engagements until further advice. We will get back to you on Friday, July 15, 2022 as well as those with other health concerns.

For emergency cases, please proceed to any nearby medical facilities.

Please be reminded of the following dates for the vaccination schedule for January 2022.

Minimum safety protocols will be strictly implemented. Please always wear your face masks and face shields and
observe social distancing while inside the Institute. Don’t forget to bring your vaccination cards, identification cards, ball pens
and alcohols. Cutoff time is from 8am to 11am in the morning and 1pm to 4pm in the afternoon. Venue will be at the ANIMO
CENTER.

A Healthy and Safe Day Lasallians!

De La Salle Medical and Health Sciences Institute in partnership with the Local Government Unit of the City of Dasmarinas would like to offer their services in the safe administration of Pfizer CoVID-19 vaccine to our Special Health Sciences Senior High School at a service fee of P400.00 per participant for two doses. The vaccination fee comprises the entire cost of the manpower, food, supplies, andvaccination cards.
The DLSUMC vaccination team will accommodate a minimum of 400 students per day to a maximum of 750 students at the
De La Salle Animo Center.
Special Health Sciences Senior High School management may set the preferred vaccination schedule from the available
dates of November 15 -19, and 22 - 25, 2021, and must submit the names of all interested SHSSHS students by November 10, 2021,
12:00 P.M. Moreover, kindly request all interested SHSSHS students to register with the Vaccine Information Management System –
Immunization Registry (“VIMS-IR”) via this link https://forms.gle/ZhZi7bvbkvKkNCoC8 on or before the said deadline date. The LGU shall deliver the total number of Pfizer COVID-19 vaccines based on the final list of the VIMS-IR registered participants.
The Department of Health (DOH) requires the patient/guardian to accompany the minor participant throughout the vaccination
process. The Informed Consent & Assent Form - attached below, is required to be signed by the minor participant and
their guardian after the health education counseling. This form will be provided by our team during the vaccination schedule.
We are looking forward to partnering with DLSMHSI: Special Health Sciences Senior High School in providing the students
with added protection against the pandemic.

A Healthy and Safe Day, Lasallians!

Did you know that last year (2020) according to the Philippine Statistics Authority, there are about 4 million Filipinos living with Diabetes Mellitus? It was even ranked 4th among the leading cause of death among Filipino adults. So how do we prevent this number from increasing?

The Post-Graduate Interns of De La Salle University Medical Center under the Department of Family and Community Medicine and in cooperation with DLSMHSI School Clinic through VITAMIN:H.S.I, invite you students and staff to a public health lecture they have prepared entitled: "Diabetes ating Talunin, (Let's) DiaBEAT-IT". Together let us beat diabetes through proper awareness and increased knowledge about this.
