๐ Angiotensin Receptor Blockers ๐
Patients with hypertension draw increased concerns of having other pre-existing health conditions. A case in point is Angiotensin Receptor Blockers (ARBs) in treating high blood pressure and heart failure, among others. ๐
Evidence demonstrates that the activation of the reninโangiotensin system and the downregulation of ACE2 expression are involved in the pathological process of lung injury after SARSโCoV-2 infection. However, the upregulation of ACE2 expression caused by ARBs has physiologic effects believed to have potential benefits to CoViD-19 patients in recent years.
By boosting ACE2 expression, ARBs (e.g. Losartan) allow more Angiotensin 2 to be converted to Angiotensin 1-7, which reduces inflammation and dilates blood vessels counteracting the effect of the virus. A multi-centered retrospective study was conducted involving elderly hypertensive CoViD-19 patients. The study concluded that the risk of severe CoViD-19 disease was significantly lower in patients who took ARBs before hospitalization than patients who are not on any hypertensive medications.๐ช
During this time of uncertainty, there is no clinical data in humans to show that ARBs may worsen patientsโ susceptibility to SARS-CoV-2. Therefore, international societies recommend that the routine use of ARBs should not be discontinued nor be influenced by speculations on CoViD-19 infections.
