ADMISSION GUIDELINES(read carefully)
1. Schedule an appointment by phone or email before coming to the facility. We currently do-not entertain walk-in inquiries (NO schedule-NO entry).
2. For inquiries about our services and rates, please check the website under FAQs page.
3. For other questions, please call, text or send us a message through email or through our social media accounts (Facebook/Instagram) and we will get back to you as soon as possible.
4. Once your schedule is confirmed, you have no less than 24 hours to cancel or reschedule an appointment. No show (1 time) or rescheduling (2 times) of a client/patient without a valid reason will black list the individual from using the facility.
5. On site: Before entering the facility, please fill-up the ‘COVID 19 Screening Checklist’ and the ‘House Rules Forms’, sign and submit for clearance.
6. Once cleared, affix your signature on the attendance logbook.
7. For patients, make sure all your vital signs are checked before, during and after the session. Please submit pertinent documents regarding your case.
8. For patients and clients, please let us know of any medical condition/s and precautions.
9. For patients and clients, please let us know anytime during the session if you are not feeling well.
10. At the end of the session, you may schedule your next appointment and be sure to ask for an official receipt from us.
HOUSE RULES(read carefully)
1. NO schedule – NO entry
2. NO mask and face shield – NO entry (keep them on at all times)
3. PUNCTUALITY – 15 minutes late from your schedule is considered as an ABSENCE = NO admission (re-schedule by phone/text message/email)
4. ONE patient – Strictly ONE caregiver policy (other family members, driver, etc. are not permitted inside).
5. Personal training clients are not allowed to bring company.
6. Disinfect your hands thoroughly before you enter.
7. Wipe your footwear on the rag thoroughly before entering.
8. Personal care-givers/nursing assistants must wear gloves at all times.
9. NO food/beverages allowed inside except water.
10. Bring your own towel and change of clothes
11. Bring your own pillow for patients who regularly use the therapy mats for exercises.
12. Plastic bags are provided inside the restroom for soiled diapers and other trash (please seal them and bring them home).
13. Wash/Disinfect your hands thoroughly after using the restroom.
14. Keep the conversations to a minimum during your session.
15. NO phone allowed for personal training clients when in session.
WE RESERVE THE RIGHT TO REFUSE ENTRY OR SERVICE TO ANYONE AT ANY TIME WHO FAILS TO ABIDE BY THE RULES OF KILOS DAVAO.
RATES(read carefully)
FOR THERAPY/REHABILITATION MEDICINE
-EVALUATION/CONSULTATION/ROUTINE VISIT: Php 1,000.00
-6 SESSIONS: Php 4,800.00
-12 SESSIONS: Php 8,400.00
FOR PERSONAL TRAINING
-CONSULTATION: Php 1,300.00 per session(online or in-person)
-SESSION: Php 1,000.00 (online or in-person)
-6 SESSIONS: Php 4,800.00 (Not allowed to re-schedule appointmnet. No show or absences secondary to unpunctuality will be deducted from your chosen package)
-12 SESSIONS: Php 8,400.00 (Only allowed to re-schedule appointment 1 time, no show or absences secondary to unpunctuality will be deducted from your chosen package)
-72 SESSIONS: Php 45,000.00 (Only allowed to re-schedule appointment 4 times, no show or absences secondary to unpunctuality will be deducted from your chosen package))
Partner Training (2 people) 1-hour SESSION: Php 800.00/person
Online Coaching (program-based/not live session)
-First 2 months: Php 5,000.00 per month
-Every succeeding month: Php 4,500.00 per month
Customized Training Program(sent by mail daily)
Chat Support and Check-ins(weekly live video calls)
Nutrition Protocol(macro-based)
1 complimentary session/month
Online Coaching (one on one 1 hour live session-ideally with basic gym home set-up)
-First month: Php 7,500.00 per month (12 sessions + 1 complimentary session)
-Every succeeding month: Php 7,000.00 per month (12 sessions + 1 complimentary session)
Customized Training Program during and in-between training sessions (sent by mail in-between days)
