Mechanics:
1. Magresgister lamang sa form na ito - https://forms.gle/D6iCwZFGSBjMkcG96

2. I-fill up ang form at magpakita ng proof of purchase (resibo o photo) ng kahit anong RiteMED product/s para makapagregister.

3. Antayin ang ibibgay na patient queue number at appointment schedule. Ang programang ito ay first-come-first-serve basis.

4. Hanggang 50 na pasyente kada araw mula 9:00 am - 6:00 pm lamang ang mabibigyan ng appointment para sa libreng pakonsulta sa doktor.

Kaya magpakonsulta na, dahil ang gusto namin ay gumaling kayo!
