TERMS AND CONDITIONS:
house call / home visit:
the service is available in: sorsogon city area only and within sorsogon Province accessible by road and minimal walking distance from the road.
service available on:
mondays----5pm to 10pm only
tuesdays-----5pm to 10m only
wednesdays----5pm to 10pm only
thursdays----5pm to 10pm only
fridays----5pm to 10pm only
saturdays and sundays by appointment only subject to availability of my schedule.
Professional fees as ff:
1. within sorsogon city proper P1000.
2. within sorsogon province outside sorsogon city proper P2000.
3. Within Sorsogon Province on far flung area P3000.
Note:
1. all emergencies should be brought to the nearest Hospital's Emergency Room, this service is strictly for non emergency consultation only.
2. Those who are under the care of a specialist, should go to their respective specialist doctors.
3. All services in house visit are strictly for Medical Consultations only, we do not cater ambulance service or transport of patients, please refer to proper ambulance conduction services.
3. services involve the advise upon assessment of the patient, to be referred to specialist or admission to hospitals, or advise for laboratory examinations, and not necessarily only prescriptions, which may or may not be given, depending on the case of the patients. So please whether a prescription/and or an advise is given, the doctor is entitled to his/her corresponding professional fee stated above.
4. Foley Catheter insertion can be permitted to be done provided that the patient have already consulted a urologist, and only for change of catheter, all new insertions should be done in hospital settings under a doctor/specialist supervision, and also only if sufficient reason that no urologist is available to cater the service of re-insertion of indwelling folley catheter.
5. all NGT insertions either new or change should be done no less than in emergency room of a hospital, as this procedure cause certain risk.
6. No Medico-legal cases please.
