Bringing care closer to home 🏡

These photos capture a series of home visits conducted throughout the month of March in Brgy. San Jose, Koronadal City for our elderly and bedridden patients—especially those living in far-flung areas with limited financial means and physical capacity to travel to health centers. Through these visits, we aim to reach individuals who are often left behind, ensuring they continue to receive the care and attention they deserve.

As part of our community rotation, Dr. Dionaldo recognized the need to extend healthcare beyond clinic walls. Many of these patients are elderly, bedridden, and living with chronic conditions such as Stroke and Parkinson’s disease—individuals who have become lost to follow-up due to distance, cost, and lack of accessible transportation. Through home-based care, we are able to reconnect them with essential medical services, monitor their conditions, and support their families in caregiving.

This initiative reflects the heart of Family and Community Medicine—meeting patients where they are, understanding their lived realities, and delivering holistic, continuous, and compassionate care. It is not only about treating illness, but about restoring dignity, improving quality of life, and strengthening the bridge between healthcare systems and the communities they serve.

More home visits will continue as we strive to make healthcare accessible for all—especially for those who need it most.

Healthy Mom, Healthy Baby đź’•

The Department of Family and Community Medicine Resident Physicians of South Cotabato Provincial Hospital, together with Ma’am Jenilym Bu-an Dela Cruz of POPCOM/Commission on Population and Development South Cotabato, successfully conducted a series of activities last March 20, 2026 at Brgy. San Jose, Koronadal City.
The event featured a Mother’s Class, prenatal check-ups, and a lay forum participated in by mothers and their partners—highlighting the importance of pregnancy tracking, regular prenatal care, and informed family planning.
Part 2 of this initiative was held last March 23, 2026, focusing on teenage mothers in the community. This session aimed to address their unique needs and concerns as a special population, empowering them with knowledge and support for a healthier pregnancy journey.
This meaningful activity was made possible through the active participation and unwavering support of the Barangay Health Station team—midwife, nurse, BHWs, and navigators—working together under one shared goal:
“Healthy mommy; healthy baby.”

The South Cotabato Provincial Hospital – Department of Family and Community Medicine extends its heartfelt gratitude to DOLE Stanfilco Dole PH for the opportunity to take part in the Stanfilco Health Aid & Outreach Program.

We celebrate the beautiful legacy of Dr. Mary Dana Boncavil as she begins a new and well-deserved chapter of retirement.

For generations of residents, Dr. Dana has been far more than a teacher or mentor, she has been a mother in our training. With patience, wisdom, and a deeply kind heart, she nurtured not only our knowledge but also our growth as compassionate physicians. Her gentle guidance, thoughtful words, and unwavering support helped shape who we are today.

In every lecture, every patient encounter, and every moment of quiet encouragement, she reminded us that medicine is not only a science but also an act of service, empathy, and humanity. Through her example, we learned that caring for patients means caring with both skill and heart.

And of course, we will never forget the snacks that filled not only our hungry tummies during long hours of duty days, but also our souls, small but heartfelt gestures that reminded us we were always cared for, just like daughters and sons, like a family.

Thank you, Dr. Dana, for being our guide, our inspiration, and our mother in training. Your kindness, dedication, wisdom, and warmth will always remain with us.

Bringing healthcare closer to the community.

Dr. Dionaldo from the Department of Family and Community Medicine of South Cotabato Provincial Hospital conducted a home visit in Purok Lower Supon, Barangay San Jose, for an elderly patient who has difficulty walking and traveling to the Barangay Health Station due to her current medical condition.

Through the initiative of the Barangay Midwife and the entire BHS team, we continue to bring essential health services directly to patients’ homes. More home visits will be conducted for residents in Barangay San Jose who are in need of care.

Remembering the Life and Strength of Nanay Flor

Nanay Flor will always be remembered as a loving mother, a devoted grandmother, and a woman of quiet strength and courage. Through the many challenges she faced in her illness, she continued to show remarkable resilience, faith, and love for her family. Her journey was a reflection of a mother’s enduring strength always fighting, always caring, and always holding on to the people she loved the most.

The South Cotabato Provincial Hospital – Department of Family & Community Medicine Palliative & Hospice Team extends our heartfelt sympathy and prayers to the family of Nanay Flor.

It was an honor for the SCPH Palliative and Hospice Care Team to walk alongside Nanay Flor and her family during this time. In palliative and hospice care, we are reminded that caring goes beyond the patient, it embraces the family, the shared memories, and the love that surrounds each moment.

The South Cotabato Provincial Hospital Department of Family & Community Medicine-Palliative & Hospice Team extends our heartfelt sympathy and prayers to the family of Nanay Petra, as we remember and give thanks for her 101 beautiful years of life.

Nanay Petra lived a life filled with love, faith, and devotion to her family. For more than a century, she was a wonderful mother, a cherished grandmother, and a source of warmth and guidance to those around her. Her gentle presence and loving heart created a home filled with care, kindness, and treasured memories that will continue to live on through the many lives she touched.

Si Nanay Petra ay namuhay nang puno ng pagmamahal, pananampalataya, at pag-aaruga sa kanyang pamilya. Sa loob ng mahigit isang siglo, siya ay naging mapagmahal na ina, butihing lola, at gabay ng kanyang pamilya.
