Case Summary

A middle-aged female patient with morbid obesity and a known history of gallstones presented with acute onset right lower abdominal pain. Clinical evaluation and imaging were consistent with a diagnosis of acute appendicitis. Given her concurrent symptomatic cholelithiasis, a combined surgical approach was planned.

The patient underwent laparoscopic appendectomy and laparoscopic cholecystectomy using a standard 3-port technique. Port placement included:

10 mm umbilical port (camera port)

10 mm epigastric port (working port)

5 mm left upper abdominal port (additional working port)

The procedure was completed laparoscopically without intraoperative complications.

Stone retrieved from cystic duct

Retrograde laparoscopic cholecystectomy performed

Cystic duct ligated using an extracorporeal Roeder’s knot
