Acute Watery Diarrhea Outbreak Response (Suspected Cholera)
Location: Wardak, Union Council Jandri, Tehsil & District Karak

1. Background
On 12 August 2025, Dr. Mohammad Rafiq (Medical Specialist) and Dr. Maqsood (DMS, DHQ Hospital KDA Karak) reported an outbreak of suspected cholera cases from Wardak, Union Council Jandri, Karak.
Upon direction of the Honourable DHO Karak, Dr. Muhammad Shoaib, the outbreak response team was immediately notified and mobilized.

3. Hospital Investigation
The team visited the medical ward and investigated 08 admitted patients, all admitted the previous day with severe dehydration.
3 patients had presented in shock on arrival.
All patients were stable and under treatment at the time of visit.
Stool samples were collected from 05 patients for cholera RDT testing – all tested negative.
Feedback on patient status and lab results was provided to the Medical Specialist and DMS.
4. Field Investigation & Community Response
The team visited the affected area in Village Wardak:
49 patients were examined for symptoms of acute watery diarrhea.
Treatment provided: Oral medications, ORS sachets.
Health education given on oral rehydration, hand hygiene, and safe drinking water practices.
All patients were stable at the time of visit.
5. Environmental Assessment
The main water source was found to be an uncovered pressure pump.
Instructions were given to cover and protect the source to prevent contamination.
Preventive measures and safe water handling were explained to the community.

7. Conclusion
The outbreak was promptly investigated and managed. While RDT results for cholera were negative, the symptoms and clustering indicated acute watery diarrhea likely linked to contaminated water from an uncovered pressure pump. Community awareness and preventive interventions were implemented to control further spread.
