Patient presented with neurogenic claudication and progressive weakness in both lower limbs which was not responding to all conservative measures.
MRI shows disc with L5 S1 lesthesis.
The only successful approach in this patient is disc removal with pedicle screws fixation. Only disc removal surgery will worse the condition and not recommended in these patients.

Today case of spine Tuberculosis on D5 dorsal level with paraplegia (complete loss of power in both legs 0/5).
Fixation and decompression done with drainage of pus which was sent for Gene Expert and culture.
So home carry massage is that TB is still a serious threat to health in our region and can cause death paralysis and other comorbitities.
So timely diagnosis and treatment is necessary.

Patient with mild disc bulge and otherwise normal MRI using medicine from years without any improvement and now with decreasing power in lower limbs(neurological deterioration).
On exploration unstable spine .
So every normal MRI is not indicating normal patient.

Today huge brain tumor surgery . Gross total resection was done and sent for histopathology analysis.
Alhamdulellah pt is allert and fully responsive after surgery and shifted to ICU without ETT.

Today patient with backache and bilateral sciatica from last one year. Now also developed neurogenic claudication. MRI disc with stenosis but perop there was fracture and tilting his of L5 facet on right side with instability.
Discectomy with decompression was done with pedicle screws fixation on L4 L5 and S1 levels.

This pt was having progressive paraparesis with power 2/5 and numbness from last one year. MRI of cervical spine showing mass with destruction of C7 vertebral body and posterior compression of spinal cord on that level. C7 corpectomy was done and cege was placed and fixed with plate and screws.
The lesion seems to be infective in nature due to Frank puss as can be visualize in pics, but also same was sent for histopath analysis also.

Today surgery of Ulnar nerve exploration above and below the elbow joint which was injured in a Road traffic accident one year back and no sign of recovery/improvement. During procedure and exploration a big neuroma was identified and was entrapped inside the injury which was dissected and main nerve was free.nerve was placed in it's groove (medial epicondyle of elbow).Total surgical procedure went uneventful. Nerve was checked after surgery via movement of fingers. pt was shifted to ward.

End plates of L5 and S1 vertebrae are eroded and vertebrae are slipped over each other as visible on MRI also which was fixed with pedicle screws fixation.

Today surgery in FC hospital
Patient was having neck pain with numbness and weakness both upper limbs with power 3/5.
MRI cervical spine C5 C6 lesthesis with cervical disc and thickened ligamentum flavum which was compressing the spinal cord.
Patient was operated .
C6 vertebral body was removed with removal of C5 C6 and C6 C7 discs and removal of thickened ligamentum flavum. Cervical cage was placed and fixed via Titenium plate and screws.
Soon after surgery patient was pain free and his power in both arms was normal 5/5.
Pt was shift to ward with cervical collar.

Patient with meningitis complicated by hydrocephalus with infected CSF which was treated initially via EVD till clearing his CSF and today followed by VP shunt.
