Science Explained: Fertility in Men vs Women

Men remain fertile from puberty until the end of their lives, producing millions of s***m every day.

In contrast, women have a finite fertility window, beginning at puberty and ending with menopause.

Women are born with approximately 1 million eggs in the form of ovarian follicles, but they cannot produce more eggs throughout their lifetime.

Each month, only one egg is released during ovulation, and it remains viable for 12 to 24 hours, while s***m can survive in the female reproductive tract for 2 to 3 days. Male fertility is continuous, with an estimated 1,500 s***m produced every second and up to 8 billion s***m regenerated by the end of a full s***m production cycle.

Fertility declines with age for both men and women, though it happens earlier for women. Female fertility begins to decrease after 30, with a 25% chance of conceiving naturally under 30, dropping to 20% after 30, and to just 5% by age 40.

Male fertility also declines, typically starting around 40 to 45 years, due to reduced testosterone levels and decreased s***m quantity and quality. Infertility affects both genders, with female-related issues responsible for 40% of cases, male factors accounting for 30%, and both partners involved in 20% of cases.

Factors like weight and smoking also play significant roles in fertility; 12% of female infertility is linked to being overweight or underweight, and smoking accounts for 13% of female infertility. Genetic factors further influence fertility in both men and women, impacting egg supply in women and s***m production in men.

*Intrauterine Insemination (IUI) is a fertility treatment that involves placing s***m inside a woman’s uterus to facilitate fertilization. The goal of IUI is to increase the number of s***m that reach the fallopian tubes and subsequently increase the chance of fertilization.

IUI provides the s***m an advantage by giving it a head start but still requires a s***m to reach and fertilize the egg on its own. It is a less invasive and less expensive option compared to In Vitro Fertilization.

When is IUI used?
The most common reasons for IUI are low s***m count or decreased s***m mobility.
However, IUI may be selected as a fertility treatment for any of the following conditions as well:

Unexplained infertility
A hostile cervical condition, including cervical mucus problems
Cervical scar tissue from past procedures which may hinder the s***ms’ ability to enter the uterus
Ej*******on dysfunction
IUI is not recommended for the following patients:

Women who have severe disease of the fallopian tubes
Women with a history of pelvic infections
Women with moderate to severe endometriosis

How does IUI work?
Before intrauterine insemination, ovulation-stimulating medications may be used, in which case careful monitoring will be necessary to determine when the eggs are mature. The IUI procedure will then be performed around the time of ovulation, typically about 24-36 hours after the surge in LH hormone that indicates ovulation will occur soon.
A semen sample will be washed by the lab to separate the semen from the seminal fluid. A catheter will then be used to insert the s***m directly into the uterus. This process maximizes the number of s***m cells that are placed in the uterus, thus increasing the possibility of conception.

The IUI procedure takes only a few minutes and involves minimal discomfort. The next step is to watch for signs and symptoms of pregnancy.

What is IUI’s Success Rate?
The success of IUI depends on several factors. If a couple has the procedure performed each month, success rates may reach as high as 20% per cycle depending on variables such as female age, the reason for infertility, and whether fertility drugs were used, among other variables.
