Magic of Ilizarov
Chronic osteomyelitis is a disabling infection of bones..
This patient is 16 years old went with me through very long journey of debridements, sequesterctomy (about 4 times), over more 2 years..
Complicated with complex deformity as a sequelae of osteomyelitis of right tibia :
1-Varus..
2-Internal tibial torsion
3- procurvatum of the tibial shaft
Management sequence:
Corticotomy=> varus correction , procurvatum and then torsion
Results after 6 months:
Straight limp , almost infection controlled with bone consolidation , with residual proximal tibia varus planned to be corrected later.

Ilizarov give the solution for difficult cases & with good planning & patience the results are magical..

16 years female exposed to explosive missile in 1 year ago ..
Sustained open fracture of the proximal femur , treated there with ex fix to manage wound healing..
She managed to go out the unsafe situations after wound healing but with deformed limb..
O/E : LLD 6 cm ,
large scar anterolateral proximal
thigh with soft tissue loss
Limited hip ROM
Normal NV exam
Plan : Valgus closing wedge corrective osteotomy -/+ limb lengthening
Post operative limb measurement was about 2.5 shortening , but she was stand with about equal legs & started partial weight bearing happly and decided not to strat distraction.

A kid with chronic osteomylitis, a story of reconstruction..
Magic of Masquelte technique & Ilizarov fixation..
He was presented with exposed proximal tibia with discharge of moderate amount of pus & a year of bed ridden & multiple debridments..

Management :
* debridment + sequesterectomy & bone cement loaded antibiotic spacer
* 6 weeks later,, wound healed, inflammatory markers normalized,.. Planed for 2nd stage =
Bone graft & Ilizarov fixation
After 7 months he was ready to remove the frame & walking happily independent.
