Finally settled down today on the first Sunday.

Woke up on 1st January 2026 with an urgent phonecall. A major heart attack patient in a remote place overseas with a cardiac arrest.

Limited resources for onsite with no emergency angioplasty capabilities.

Requested to be transferred to Singapore 🇸🇬 under my care for advanced treatment.

What followed has been an intense period of judgement making and evacuation planning with the hospital team from both ends and with the medical evacuation crew.

I had to fall back and order something I have not used since my early training days - systemic thrombolysis to break up obstructive blood clots in the culprit coronary artery.

Risk of a major bleeding in the body but there was no other way. Calculated decisions.

Managed to get a good outcome, flew in late last night 3rd Jan 2026 as per the jet availability and performed the angioplasty first thing today morning.

And my hospital 🏥 team told me that it was the first angioplasty for the year. Interesting to see my sole name on the log book.

Truly a fullfilling lifesaving start to 2026. It is hard not to get emotional with the family who are so very relieved.

It is amazing how the balloon 🎈 technology has evolved in coronary angioplasty to dilate the arteries before the optimal implantation of drug eluding stents or drug eluding balloons.

Calcium (hardened plaques in arteries) remain the most challenging to overcome. Thick deposits (yellow outline) require substantial pressure to effect cracks to break them.

Drills (artherectomy) help shave the deposits to reduce the burden but do pose risks to patients.

Dr Pinakin recently performed coronary angioplasty in a heavily calcified artery in a frail patient using a novel balloon studded with metallic beads.

The multiple beads are meant to compress on the calcium under pressure to crack it (hard on hard concept).

A satisfying end result with no drills needed in this patient.

Shared my thoughts and experience with The Straits Times in an article today with regards to the recurring risk of further heart attacks following an index one.

Liked the word play "Heart Truths". Patients are often worried what lies ahead and sometimes hard truths are the best way to give them all the information to make a change.

Coincidentally, the date today also reflects a change in Singapore 🇸🇬. 15th May 2024. New Prime Minister. Looking ahead for our next chapter!
