[Rama CO-RADS]

: ใช้กับ Chest X-RAY
: ใช้เป็นแนวทางในการรายงานผล CXR ที่สงสัย COVID-19 เพื่อให้ง่ายต่อการสื่อสารและเพื่อ triage คนไข้ที่ hospitel หรือรพ.สนาม

Category 1: Normal CXR หรือไม่พบความผิดปกติ

Category 2: Minor abnormality ที่ไม่สัมพันธ์กับ COVID-19 pneumonia
: Anatomical variant, breast implant, scoliosis
: Technical issue e.g. suboptimal inspiration, off-center exposure แต่ไม่ส่งผลต่อการแปลผล
: ความผิดปกติที่ไม่สัมพันธ์กับ COVID-19 pneumonia และไม่จำเป็นต้องรักษาตอนนี้ e.g. old TB, cardiomegaly, aortic atherosclerosis, etc

Category C: Low probability or atypical for COVID-19 pneumonia แต่น่าจะเป็น clinically significant disease อื่นๆ ซึ่งต้องใช้ clinical correlation หรือ further management
: สงสัยโรคอื่น e.g. bacterial pneumonia, active TB, congestive heart failure, pneumothorax, pleural effusion, malignancy, etc.

Category 3: Equivocal/unsure/indeterminate for COVID-19 pneumonia
: เห็น findings ที่อาจจะเป็น early/mild/atypical COVID-19 pneumonia หรือจากสาเหตุอื่น(e.g. pseudolesion, other diseases) ต้องไป clinical correlation, follow up หรือ repeat CXR เช่นเจอ subtle, poorly defined opacities

Category 4: Suspicious for early/mild COVID-19 pneumonia
: Single or multifocal unilateral poorly defined ground-glass opacities

Category 5: Typical for COVID-19 pneumonia
: Multifocal bilateral peripheral opacities
: Opacities with rounded morphology

[CO-RADS]

: CT-based system สำหรับประเมินความน่าสงสัยในการมี pulmonary involvement of COVID-19 ใช้กับผู้ป่วย PUI

CO-RADS 0: incomplete or insufficient quality

CO-RADS 1(PCR +ve 6%): Very low level of suspicion
- Normal CT
- Findings ที่เห็นชัดเจนว่าไม่ใช่ infection e.g. emphysema, tumor, fibrosis, perifissural nodule
- Typical interstitial pulmonary edema with pleural effusion

CO-RADS 2(PCR +ve 14%): low level of suspicion
- Findings เป็น typical infectious disease ที่ไม่เหมือนกับ COVID-19 e.g. bronchiolitis, bronchopneumnomia, lobar pneumonia, lung abscess.
: Tree-in-bud sign
: Centrilobular nodular pattern
: Lobar or segmental consolidation
: Lung cavitation
- สามารถเป็น atypical appearance of COVID-19 ได้

CO-RADS 3(PCR +ve 34%): equivocal findings
- Findings ที่เจอได้ใน viral pneumonia อื่นหรือ non-infectious cause
: Perihilar ground-glass opacity
: Homogenous extensive ground-glass opacity with or without sparing of some
secondary pulmonary lobules
: Small ground-glass opacities that are not centrilobular or not located close to the visceral pleura
: Ground-glass opacity together with smooth interlobular septal thickening with or without pleural effusion in the absence of other typical CT findings
: Consolidation compatible with organizing pneumonia without other typical findings of COVID-19

CO-RADS 4(PCR +ve 84%): high level of suspicion
- Findings typical for COVID-19 แต่ก็เจอใน viral pneumonia อื่นได้
- เห็นเหมือน CO-RADS 5 แต่ไม่ contact visceral pleura, เจอ unilateral, peribronchovascular distribution หรือ superimposed กับ severe diffuse lung abnormality ที่มีอยู่เดิมแล้ว

CO-RADS 5(PCR +ve 93%): very high level of suspicion แบ่งเป็น 2 แบบ
1. พบ obligatory findings เป็น multifocal bilateral GGOs with or without consolidation ใกล้กับ visceral pleura or fissure
2. พบ confirmatory findings ด้านล่าง + มีการ evolution ตามลำดับข้างล่าง
: Multiple GGOs มักจะมีลักษณะกลมและขอบไม่ชัด แต่ถ้าขอบชัดมักจะมีรูปร่างตาม outline ของ secondary pulmonary lobules
: Crazy paving pattern
: Develops consolidation in GGOs
: Organizing pneumonia(reverse halo, GGO with extensive subpleural consolidation and air bronchograms)
: Subpleural curvilinear bands or bands of ground-glass with or without consolidation in tethered arching pattern with small connections to the pleura
- สามารถเจอ thickened vessels ข้างใน lung abnormality ใน confirmatory findings ได้

CO-RADS 6: Proven

[Abdominal findings of COVID-19 แบบย่อๆ]....

เชื้อไวรัส SARS-CoV-2 ซึ่งก่อให้เกิดโรค COVID-19 นั้น แม้จะมีอาการทาง respiratory system เป็นหลัก แต่ก็มีคนไข้ราว18-34% ที่มาด้วย GI symptoms เช่น abdominal pain, diarrhea, vomiting หรือ Biliary tract injury เพราะมี ACE2 expression ใน GI tract, pancreatic islet cells และ biliary epithelium(SARS-CoV-2 เข้า cell ผ่าน ACE2 receptor)

Mechanism ที่ก่อให้เกิดโรคยังคงอยู่ระหว่างการศึกษา แต่น่าจะมาจาก 4 อย่างหลักๆ

1) Direct viral toxicity
2) Endothelial cell damage and thromboinflammation
3) Dysregulation of immune response
4) Dysregulation of renin-angiotension-aldosterone system

Imaging findings ถ้าเอาสั้นๆ คือ inflammation and/or ischemia ของ organ ต่างๆ
....

มาเริ่มกันทีละ organ ดีกว่า

1) Small bowel and large bowel
: เป็น extrapulmonary manifestation ที่เจอบ่อย บางครั้งเจอก่อน pulmonary manifestation ด้วย
: Typical findings มี bowel wall thickening, wall edema, bowel dilatation, pericolic fluid or fat stranding, regional lymphadenopathy ->ทุกอัน non-specific
: เจอ findings ของ bowel ischemia(no wall enhancement, pneumatosis intestinalis, portal venous gas) ได้โดยจะเจอ thrombosis ของ artery หรือ portal vein ร่วมด้วยหรือไม่ก็ได้
: Pneumatosis intestinalis อาจเจอโดยไม่มีอาการหรือมาแบบอาการ severe ก็ได้
: มีบางงานวิจัยให้ bowel wall inflammation + ischemia + thrombosis ใน mesenteric artery หรือ portal vein เป็น typical findings ของ COVID-19 ด้วย
: บางเคสอาจจะเห็น inflammation ของ appendix mimicking appendicitis ได้ด้วย

2) Liver
: ยังไม่มีงานวิจัยชัดเจน แต่มี report เจอ hepatic steatosis ใน CT และ US ของผู้ป่วย COVID-19 with liver injuryจากผลตรวจ Lab

3) Kidney
: Findings ที่สามารถเจอได้คือ renal infarction
: COVID-19 ทำให้เกิด acute renal dysfunction ได้ ต้องระวังก่อนฉีด contrast media ด้วย

4) Pancreas
: Findings แบบ acute pancreatitis เลย สามารถเจอ pseudocyst, WON, abscess ได้

5) Gallbladder
: Findings เหมือน acute acalculous cholecystitis ทั้ง ซึ่งอาจจะเกิดจากตัว virus หรือเกิดจากการที่คนไข้เป็น critically-ill patient
: เนื่องจาก virus ทำให้เกิด mucosal injury โดยตรง cholecystitis จาก COVID-19 อาจจะมี gallbladder distension, pericholecystitic fluid or fat stranding น้อยกว่าเคส acute cholecystitis ทั่วไป
: Complication ที่เจอได้ เช่น gangrene, perforation, fistula, portal vein thrombosis, cystic artery pseudoaneurysm

6) Spleen
: Findings ที่พบได้ คือ splenic infarction

นอกจาก Findings ตาม organ ที่ว่ามาแล้ว ก็ยังอาจจะเจอเรื่องของ bleeding จากการใช้ anticoagulant(เจอ hematoma or bleeding) หรือ hypoperfusion complex ร่วมด้วยได้

เคยฟังเพลงตอนอ่านฟิล์มกันบ้างมั้ย

แอดชอบฟังตอนอ่านพวก plain film นี่แหละ ทำให้อ่านได้เร็วขึ้นและนานขึ้น

หลังจากที่ลองฟังดูหลายๆแนว แอดชอบแนว Lo-Fi hip hop ที่เป็นเพลงบรรเลงอย่างเดียว ฟังแล้วเพลินดี ฟังไปทำงานไป แป๊บๆ อ่าว อ่านเสร็จหมดแล้ว

ความจริงมีอีกหลายแนวเลย ทั้ง Jazz, Blues, Pop, etc

ส่วนพวกเพลงแนว Deep focus ที่เค้าว่าช่วยเพิ่มสมาธินี่ ฟังแล้วรู้สึกหลอนๆชอบกล ไม่ใช่แนวเท่าไหร่
