According to responses from various stakeholders (respondents) in the survey on Nursing and Midwifery education training and practice about the need for reforms on professional education training development, we realized that the professional education training development needs for the majority of the licensed Ugandan Nurses and Midwives has not been heard and met for them to meet the needs of the community.

All the licensed nurses and midwives who responded in the survey for the need of specialised Nursing to be trained right away from diploma, bachelors and postgraduate levels picking from the pool of the Certificate (enrolled), Diploma (registered) and Bachelors (graduate) general nurses believed that it would help them not to branch or cross to other health care courses and this is a benefit to the nursing practice in order to retain skilled and experienced practicing personnel.

Therefore the following recommendation need to be looked into by the various stakeholders in Nursing education training and practice.

1. Introduction of training in Diploma nursing specialities directly picking from the pool of already existing general certificate nurses and Midwives in fields like emergency nursing, intensive (critical) care nursing, pediatric nursing, geriatric nursing, adolescent nursing, gynaecologic nursing, community health nursing, surgical nursing, palliative care nursing and so many others rather than as advanced Diplomas after a Diploma in general nursing or midwifery will greatly improve the Nursing education training and practice to meet the needs and demand of the local community in Uganda

2. Training of Bachelor of Nursing in various specialised fields of nursing as above (proposal for Diplomas), picking from the pool of diploma (registered) general nurses and Midwives can add passion for professional development for licensed registered nurses and midwives and also increase retention of Ugandan nurses in nursing practice. Uganda's Nursing and Midwifery fraternity has continuously lost experience practitioners at Diploma and Bachelors level by going for other health related courses as an escape route to mini specialization.

3. At masters training level in nursing Uganda Universities still has few graduate nurses and Midwives progressing in the core nursing fields because of the limited field options and therefore more field options need to be introduced to meet global need and demand for super specialised Nursing services.
For the better Development of Nursing practice and services in Uganda and the world at large, the various stakeholders need to promote this concept and put it in real action.

UGANDAN NURSES AND MIDWIVES.

Greetings dear colleagues.
We have been called to serve humanity with a positive and selfless attitude and I believe we are trying our best.

I have observed avery slow if not stanted growth in the Nursing and Midwifery training and practice here in Uganda.

I believe these noble professionals are of great value in the health service delivery structure but some of them do not know their value and only cost themselves in monetary value.

Professional development for Nurses And Midwives has been a mockery issue which needs to be handled with sober minds:
1. The horizontal education development of double training is a deliberate act by stakeholders to keep Nurses And Midwives mind stanted at the same level of education and salary scale.
Why should one do a diploma in Nursing or Midwifery to qualify for another diploma in Nursing? Eg pediatrics, community health nursing, anaesthesia etc why can't it be made a direct diploma after certificate in Nursing or Midwifery or A bachelor speciality after a diploma in Nursing or Midwifery?

2. Lack of Nursing training specialities in Nursing at Diploma and Bachelors is a deliberate action to a have a pool of Nurses And Midwives with general limited knowledge. Why can't the the Nursing and Midwifery education fraternity introduce Nursing specialities at Diploma and Bachelors level to pick directly from the pool of already existing general certificate and diploma Nurses And Midwives to promote vertical Nursing and Midwifery education coupled with speciality professional development?

Who is behind this disorganization and Who benefits out of it?

What is the role of a licensed Nurses And Midwives of Uganda in promoting vertical and specialised Nursing and Midwifery training/education in Uganda?

What is the role of Uganda Nurses And Midwives Union in promoting specialised Nursing and Midwifery training and vertical Nursing and Midwifery education?

What is the role of Uganda Nurses And Midwives Council in promoting specialised Nursing and Midwifery training and vertical Nursing and Midwifery education

It's high time we start a Nursing and Midwifery education and training revolution in Uganda for better specialised Nursing services which are provided by accredited licensed specialised Nurses And Midwives.

PUBLIC HEALTH CONCERN.
KASESE MUNICIPALITY
STAND WARNED.

A very serious and dangerous common practice has been observed among majority of the citizens of Kasese municipality especially Nyamwamba division where RNAMIDA office is located.

Whenever it rains, they dump their domestic waste in water drainage channels and even little children play in these flowing garbage filled water channels and at the end of it all, there is also residue waste retained in these channels, which has far reaching out comes:
1. Risk for immediate loss of children's lives due to drowning
2. Increased risk of contaminated water related diseases liked cholera, typhoid, etc
3. Risk for floods due to blocked water drainage channels
4. Environmental pollution.
5. Road destruction due to interrupted water flow which washes away the marram in the roads
6. Displacement of some market vendors especially those in Kizungu market leading to economic paralysis.
7. And many other health and economic outcomes.

As RNAMIDA we therefore recommend that authorities take action at their various levels of management and enforcement including but not limited to:
1. Continuous sensitization of the community members about safe waste disposal practices.
2. Kasese municipality domestic waste management unit need to evacuate waste as often as possible during the rainy season to avoid heaping of waste in homesteads and also waste collection points in public places like markets.
3. Parents be sensitized on keeping an eye on their children when ever it rains.

NURSES AND MIDWIVES ARE THE HEART OF HEALTH CARE.

Nurses and midwives are the heart of all health care systems in the world.

Without Nurses and Midwives the health system can not even survive for a microsecond.
In developed countries, Nurses And Midwives are applauded and highly respected for their noble contribution and support towards health care.

In Africa, the contribution of nurses and midwives is partially recognized and this is a key reason why Africa's health systems are almost in coma.

In Uganda, I have seen some other health care professionals brag over Nurses and Midwives just because Nurses and Midwives in Uganda are like the crested crane and the kob on the national court of arm, in that they have not really recognized their value and this affects their attitude towards their profession and tittle which affects the quality of work and health care as a whole.

In Uganda there is need for reforms in nursing and midwifery training, scope of work and renumeration.
There's need for a national open medal awarding process and ceremony to motivate Nurses And Midwives in Uganda.
There is need for embracing more specialised nursing training even at least at a diploma level to enroll right away from certificate in nursing. It's a good but not economical idea to opt for horizontal education (two diplomas) to create low level nursing specialists.

Ugandan Nurses And Midwives it's high time you voice out the best way forward to improve and develop our profession.

I call upon the stakeholders for Nursing and Midwifery training, and practice in Uganda to look through the various loop holes in order to improve our health care systems if our country is to develop.
