We are proud to announce the release of the 2025 Community Health Needs Assessment (CHNA) for Greater Bridgeport - a vital tool that highlights the most pressing health needs within our communities. Over the course of 18 months, we set out to hear your insights and collect data on local health trends and disparities to identify opportunities to improve wellbeing for everyone across our region.

After identifying the top 25 health needs, we engaged in an in-depth process with community members and partner organizations to prioritize the areas where we could best focus our efforts: Mental Wellbeing; Preventive Care and Quality of Life; and Strengthening Communities. These three priority areas and our plan to address them, known as a Community Health Improvement Plan (CHIP), are detailed in the full report.

This report is backed by data, informed by your voices, and designed to drive action. Whether you are a community member, policymaker, community advocate, or health professional, this report is for you. Partnering to Build Healthier Communities Since 2003

In 2003, the Health Improvement Alliance (HIA) was formed to help address the health needs of the Greater Bridgeport community, which includes the six towns of Bridgeport, Easton, Fairfield, Monroe, Stratford, and Trumbull. In early 2020, HIA expanded to a seven town region and now also includes the town of Milford.

Initially named the Primary Care Action Group (PCAG), the coalition changed names in 2019 to HIA to more accurately reflect the expanded focus of our work. HIA is a coalition of two neighboring hospitals, Bridgeport Hospital and St. Vincent’s Medical Center, along with the seven departments of public health, federally qualified health centers, and about 50 community and non-profit organizations all serving the Greater Bridgeport Region.
