Nurses provide coordinated skilled nursing care to pediatric and adult patients in their place of residence. Nurses will demonstrate accountability and responsibility in collaborating with the team (LVHS staff, patient/family, and referring agency) to establish and achieve patient goals and maintain high-quality patient care. These services will be performed in accordance with the physician's orders and under the supervision of the Director of Clinical Services.

Responsibilities and Duties

Qualifications and Skills
Current Licensed Practical Nurse (LPN or RN License in Ohio)
Current BLS card
One (1) year prior nursing experience - preferred
Trach Care/Vent Experience -preferred
