Help clients and caregivers understand and access services that best suit them
Screen, arrange and monitor in-home and/or other ongoing services
Assist with moving an older person (from home to retirement complex, from one state to another, from assisted-living care to nursing facility, etc.) Act as a liaison to families at a distance to oversee and plan care
Alert caregivers to potential problems and regularly report progress
Provide consumer education and advocacy services
