To my medical colleagues: Is aspiration an illusion of safety, or a worthwhile step in complication reduction? We have all seen the studies and all heard the discussions.

A recent lip filler treatment included a positive aspiration and I had one of those quiet moments that simply reaffirms why I take the extra time to aspirate. Itโ€™s a small step, but one that aligns with the safety standard I hold for my patients.

Low-risk procedures never mean no risk. Our responsibility is to balance treatment efficacy with safety.

Aspirating doesnโ€™t mean you lack anatomical knowledge or confidence. As experienced injectors, we know anatomy confidently, but we also know that individual anatomy varies. Vasculature is not always textbook. Even when I feel confident in my placement, I still aspirate.

Is aspiration 100% reliable? No, and I agree with that completely. Sensitivity can be affected by a multitude of factors, but here is what I believe: it doesnโ€™t harm the injector or the patient, and the few extra seconds spent aspirating are far better than spending hoursโ€”or monthsโ€”managing a vascular occlusion.

I am always continuing to read, research, and listen and I genuinely value respectful discussion on this topic.
