We are now hiring a full-time day shift LPN to provide home health nursing care and other support services to patients and families Monday through Friday. Must have a high school diploma or equivalent and current LPN certification. Must be a graduate from an approved school of practical nursing with a current unrestricted WV license to practice as an LPN, and a minimum of one year experience, preferably with home health or hospice patients.

Home health is designed to help the patient rest, recover, and receive treatment in the comfort of their own home or residence. Services are provided with the goal of helping the patient regain independence to become as self-sufficient as possible while also managing their disease or condition.

Home health care can benefit both acute and chronic conditions, including but not limited to the following:
• Dementia related disease
• Kidney disease
• Chronic obstructive pulmonary disease (COPD)
• Diabetes
• Heart disease
• Recovery from illness or surgery
• Stroke/Neuromuscular disease
• Medication Management

In addition to providing medically necessary services for the treatment of the above conditions, home health care professionals act as a liaison between the patient, the patient's family, and the patient's doctor. Home health care professionals keep a log for each visit and provide updates on the patient's condition to the doctor as needed. This helps to ensure continuity of care.

We’ve noticed people can get confused when we discuss “home health care,” which is significantly different than personal “home care.”

While the two services may sound similar and both take place at a patient's home or residence, home health is administered by licensed medical professionals. Further, home health covers a myriad of ailments and diseases, providing physical therapy, post-operative care, and treatment of Alzheimer's, dementia, and chronic health conditions.

Home health does not provide unskilled services or daily necessities such as cooking, cleaning, bathing, and transportation. These services are covered by home care assistance. Some home health care services may offer personal home care assistance at an additional cost, which may or may not be covered by insurance or Medicare.

Plan to stop by our tent and learn more about Home Health of West Virginia at this year’s Westside Community Health Fair this Saturday, September 6 at Magic Island in Charleston.

A variety of local and state healthcare and social service providers will be on hand to offer wellness information and products, along with plenty of other activities, music, food and fun for the whole family.

“Now more than ever, people need assistance and don’t know where to turn for help,” said event organizer Dr. Robert L.
