Patients' Choice Home Healthcare is a family owned home healthcare agency seeking a dedicated and dependable Occupational Therapist for PRN to provide skilled occupational therapy services on an intermittent basis to our clients in their homes in accordance with the physician's orders.

We service Virginia Beach, Norfolk, Chesapeake, Suffolk and Portsmouth.

RESPONSIBILITIES:

Assists the physician in evaluating client's level of function by applying diagnostic and prognostic procedures.

Makes arrangement for outpatient services when procedures are required that cannot be given in the home.
Evaluate the effectiveness of occupational therapy intervention and updates the client care plan as needed.
Prepare clinical/progress notes on the day of the visit which are incorporated into the chart within 72 hours.

Communicate with the physician on a regular basis regarding clients' status, obtaining additional orders, if necessary.
Communicate with other team members and, when appropriate, intructs them in Occupational Therapy techniques that they may use while working with the client.

Supervise the Occupational Therapy Assistant per agency policy.
Participate in agency activities, in-services, and meetings, when appropriate.

Confirm on a weekly basis, the scheduling of visits with the Director of Nursing in order to coordinate agency visits.

Participate with the staff, the client, and they physician in discharge activities and completes an Occupational Therapy Discharge Summary.

REQUIREMENTS:

** Must be a graduate of an Occupational Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Occupational Therapy Association.
** Preferably should be a member of the American Occupational Therapy Association.
** Must be licensed/registered in Virginia

Patients' Choice Home Healthcare is seeking to add a compassionate, empathetic and team oriented home health aide to our growing healthcare team! This position is per diem with the opportunity to become full-time within the near future. We are looking for a reliable, passionate aide with a strong skill set to assist our patients within their home according to the care plan developed by the RN case manager. This position may require travel from one patient to another depending on the schedule and availability you provide. Visits will be short in nature and task driven. Compensation will be paid per visit and can be discussed in detail during the interview process. Please read the full description below for details and qualifications. DUTIES

Provides personal care and related services in the home, under the direction, instruction, and supervision of the staff nurse and the Director of Nursing.

Tasks to be performed by an Attendant must be assigned by, and performed under, the supervision of an RN who will be responsible for the client care provided by the
Home Attendant.

Under no circumstances may an Attendant be assigned to receive or reduce any intravenous procedures, or any other sterile or invasive procedures, other than re**al temperatures.

RESPONSIBILITIES

Follows the plan of care to provide, safe, competent care to the client.

Helps the client to maintain good personal hygiene and assists in maintaining a healthful, safe environment.

Plans and prepares nutritious meals, markets when instructed to do so by the nurse.

Assists the client with ambulation, as ordered by the physician, and approved, and supervised, by the nurse.

Assists the therapy personnel, as needed, with rehabilitative processes.

Encourages the client to become as independent as possible, according to the nursing care plan.

Attempts to promote client's mental alertness through involvement in activities of interest.

Gives simple emotional and psychological support to the client, and other members of the household, and establishes a relationship with client and family, which transmit trust and confidentiality.

Reports any change in the client's mental or physical condition, or in the home situation, to the staff nurse, or to the Attendant supervisor.

Performs routine housekeeping tasks as related to a safe and comfortable environment for the client, as instructed by the professional nurse.

Documents services and promptly prepares a visit report, incorporating documentation in the clinical record, weekly.

Confirms, on a weekly basis, the scheduling of visits so that other necessary visits, by staff members, can be coordinated.

Works with personnel of other community agencies involved in the client's care, as directed, by the nurse.

Attends in-service, as required by regulation.

Taking and recording vital signs, if specified in the plan of service.

JOB CONDITIONS

The ability to drive and the ability to access clients’ homes, which may not be routinely wheelchair accessible, are required.

Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care.

On occasion, may be required to bend, stoop, reach, and move client weight up to
250 pounds; lift and/or carry up to 30 pounds.

Must be able to effectively communicate in English.

QUALIFICATIONS

1. Must be able to speak, read, and write English and shall meet one of the following
6 qualifications:

a. Satisfactorily completed a nursing education program preparing for a registered nurse licensure or practical nurse licensure;

b. Satisfactorily completed a nurse aide education program, approved by the
Virginia Board of Nursing;

c. Have certification as a nurse aide, by the Virginia Board of Nursing;

d. Has been successfully enrolled in a nursing education program preparing for registered nurse or practical nurse licensure and has currently completed, at least, one nursing course that includes clinical experience involving direct client contact; or

e. Has satisfactorily passed a competency evaluation program that meets the criteria of 42 CFR 484.36 (b).

What a taxing day. Call from accreditation commission for health care at 7:30 this morning. "We are on our way". Had to ring the doorbell wake Jessie Collins up, just got in from New Orleans last night. Thought she was going to faint. Anyway first day went pretty well, thanks to great team we have in place. Another full day tomorrow and we'll see how things go.

The Speech Pathologist provides skilled speech therapy services on an intermittent basis to clients in their homes in accordance with the physician's orders, and is an integral member of the therapy team and works with the nursing staff and other therapy team members to implement the plan of care.

RESPONSIBILITIES

Evaluate the client's level of functioning and recommends mechanisms to enhance the client's hearing ability.

Provide initial and ongoing comprehensive assessments of the client's needs, including Outcome and Assessment Information Set (OASIS) assessments.

Establish a Speech Therapy treatment plan in consultation with the physician and revises it with the approval of the physician as necessary.

Assist in development and implementation of the interdisciplinary client care plan as it pertains to Speech Therapy.

Assure that the physician's orders are appropriate and discusses necessary changes.

Communicate with other team members and, when appropriate, instructs them in speech pathology techniques that they may use while working with the client.

Teach, supervise, and counsel the family and client in the total Speech Therapy program and other related problems of the client at home.

Evaluate the client, his/her significant other(s) and the home situation to determine what instruction will be required, what assistance will be available for the family in caring for the client, and what other agency and community services will be required.

Make arrangements for out-client services for procedures which cannot be given in the home.

Record on a timely basis all evaluation data, treatments, and client's response to therapy intervention.

Supervise the Home Attendant per agency policy when Speech Therapy is the only professional discipline involved in the client's care.

Record and report to the physician the client's reaction to treatment and /or any changes in the client's condition.

Participate in the development of periodic revision of the physician's plan of treatment keeping him/her informed of the client's status, and obtaining additional orders if necessary.

Communicate with the physician at least every 30 days or whenever changes occur regarding the client's status and obtains additional orders, if necessary.

Evaluate the effectiveness of speech pathology intervention and updates the client care plan as needed.

Write clinical/progress notes on the date of the visit and incorporates same in the client's record within 72 hours.

Assume responsibility for self-development by continually striving to improve his/her health care knowledge through educational programs, attendance at workshops and conferences, active participation in professional and related organization meetings and individual research and reading.

Confirm, on a weekly basis, the scheduling of visits with the Director of Therapy in order to coordinate the client's visit schedule.

Participate in agency activities, case conferences and meetings, when appropriate.

Participate with staff, clients, and physicians in discharge planning activities and completes a Speech Therapy Discharge Summary.

QUALIFICATIONS

1. A Master's Degree in Speech-Language Pathology with a Bachelor's Degree in the Field of Speech and Hearing Therapy preferred.
2. Preferably a member of the American Speech-Language and Hearing Association.
3. Licensed/registered in Virginia
4. Must have at least one year of speech therapy experience. Preference is for at least one year of experience working with adults.
5. Must have a criminal background check.
6. Must have current CPR certification.
