We are looking for a CMA/LPN to join our team as a Patient Care Manager/Chronic Care Management Nurse. In this role, you will help patients navigate the healthcare system by coordinating referrals, making follow-up calls, educating patients on medication adherence, and managing ER follow-up/hospital discharge calls.
