Day 2 of the PACH 2026 Conference brought the energy and the action! 💥

From breakout sessions to meaningful conversations, today was all about strengthening community care and building sustainable systems that truly support CHWs and the communities we serve.

Guided by the three core drivers of the conference, we leaned into:
✨ Building Power Together – fostering collaboration and lifting community voices
✨ Shaping the Workforce – investing in CHWs and strengthening the future of care
✨ Advocacy in Motion – turning ideas into action to create lasting change

We explored everything from stroke prevention tools and legal partnerships to mental wellness support and innovative care models. The keynote inspired us, the networking connected us, and the closing reflections reminded us why this work matters so deeply.

Grateful to be in a space with so many passionate leaders committed to making a difference. Let’s keep the momentum going! 💙

Day 1 at the PACH 2026 Conference in Westerville was an incredible start!

HUB staff and Community Health Workers came together from Tuscarawas Senior Center, Mediwise Pharmacy, Zanesville Muskingum Health Department, and Buckeye Hills—bringing passion, knowledge, and a shared commitment to strengthening our communities.

From powerful discussions on CHW reimbursement and care coordination to meaningful breakout sessions and networking, today was all about learning, collaboration, and moving the work forward.

There’s something special about being in a room full of people who are dedicated to making a difference every single day. Looking forward to what tomorrow brings!

Please help us welcome Kaylyn to the Bridges to Wellness team! 🎉
We are so grateful to have her serving the families and community of Guernsey County. Her passion, dedication, and heart for helping others make her an amazing addition to our CHW program.
Welcome, Kaylyn — we can’t wait to see the positive impact you’ll bring! 💙

"My name is Kaylyn Van Kirk, and I’m a Care Coordinator at the East Central Ohio Educational Service Center through the Bridges to Wellness program. I work closely with Cambridge City Schools to help bridge the gap between children, families, and the resources they need to thrive. Supporting kids and removing barriers to wellness is something I’m deeply passionate about. I’m also a proud mom of two wonderful kids and the Vice President of the local nonprofit Feed My People, where we serve free hot meals to our community Monday through Saturday from 10:00–11:30 a.m. Community care, advocacy, and showing up for others are at the heart of everything I do. "

"✨ What an amazing afternoon at NewPointe Church - Dover ✨
I had the honor of presenting at the NewPointe Church Retiree Luncheon today, sharing the mission of Bridges to Wellness and the work we’re doing to support individuals and families in our community.
It was such a joy connecting with everyone, answering questions, and hearing the heart this group has for serving others. 💛
A huge thank you to the NewPointe team for inviting me and for creating such a welcoming space. I’m grateful for every opportunity to spread the word about how we can all work together to build a stronger, healthier community.
Looking forward to continued partnership and the good things ahead! 🙌 "
