We’re excited to welcome our newest Care Coordination Agency to the Bridges to Wellness network — The Guernsey County Community Development
They have officially signed on with us and are now ready to receive referrals for individuals in Guernsey County who can benefit from supportive, community‑based care coordination services.

We just finished onboarding for the month of January!
Our first new CHW highlight is Mariela who is based out of the Cleveland Clinic Union Hospital OBGYN.
Welcome Mariela! We are so excited to have you onboard with us!! Read a little bit about her below.

Hi! My name is Mariela. I attended Buckeye Career Center Adult Education, where I earned my certification in Medical Assisting and gained hands on experience by shadowing in the healthcare field. I also worked at the YMCA as a Wellness Associate and Greeter—experiences I’m very grateful for, as they allowed me to learn a lot and connect with others. In my free time, I enjoy going to the gym and listening to music. I have also volunteered in outreach and translation services, which gave me the opportunity to support and engage with the community in meaningful ways. As a bilingual Spanish speaker, helping and supporting the community is my passion, and I am excited to continue making a positive impact as a Community Health Worker.

Bridges to Wellness has two Community Health Workers who can help any individuals with their COPD diagnosis.
We have helped with medication costs, transportation resources, utility assistance, food security, and more.

Today, our Regional Coordinator is starting Day ONE of Pathways Community HUB training! We are excited to deploy three new Community Health Workers in Tuscarawas and Guernsey Counties!
We want to welcome our newest Care Coordination Agency The Guernsey County Community Development. We are so happy for this future partnership.

Bridges to Wellness is excited to announce that we have received grant funding for a "funder of last resort" in Guernsey, Monroe, and Noble counties for 2026!

But what does that mean though?

It means our CHW's in Guernsey/Monroe/Noble can enroll any clients regardless of insurance help link them to community resources.

We are thankful for our partner United Way of Guernsey, Monroe & Noble Counties for granting us this opportunity for the 2026 year!

Currently we have three CHW's in Guernsey County and one that serves Monroe and Noble jointly.

As we wrap up this year and head into 2026, we want to thank United Way of Muskingum, Perry & Morgan Counties for being a funder of last resort for the team at the Zanesville-Muskingum County Health Department!

We are so thankful for their generosity & compassion for our participants within the community🤍

We want to extend our heartfelt gratitude to United Way of Muskingum, Perry & Morgan Counties for serving as the funder of last resort for our program.

Because of their commitment, Bridges to Wellness can continue serving participants who may not have insurance or have an insurance plan that we do not contract with.

Their partnership represents hope, stability, and a true dedication to community well-being. We are incredibly thankful for their belief in our mission and their ongoing investment in healthier, stronger communities.

We are so thankful for our Funder of Last Resort, United Way of Muskingum, Perry & Morgan Counties to help our Community Health Workers serve participants who may not have insurance or a plan that we are not contracted with!

We currently have 29 active participants being served within this grant. We are incredibly thankful for your continued generosity to our program!

Welcome to the Bridges to Wellness team, Dianne! Dianne has just finished her HUB training and onboarding and is beginning to serve participants in Tuscarawas County through the Tuscarawas County Senior Center!!

Dianne states:

"Hello, my name is Dianne Holt, and I am a new Community Health Worker at the Tuscarawas County Senior Center. Prior to becoming a CHW, I spent three years as the Lead Teacher in the Infant/Toddler room at a daycare where I developed an appreciation for nurturing growth, building trust, and supporting families. This experience taught me the power of patience, empathy, and connection. Family means everything to me, and I truly believe it takes a village to help one other grow and thrive. In my new role, I hope to bring that same sense of community to the families I serve, helping them to see that there is always hope and support available."

We are excited to see Dianne and all of her compassion for others, make a difference in our Tuscarawas County community!!💜

Welcome to the Bridges to Wellness team, Laura! Laura just finished her HUB training and onboarding and is beginning to serve participants in Tuscarawas County!!

Laura states:

"I earned a B.A. in Psychology from Ohio University in 2022. Afterwards, I moved to Columbus and conducted cancer research with OhioHealth Research Institute for several years. I made the decision to move back to Tuscarawas County and pursue something I am very passionate about: serving my community and promoting great health outcomes. The Bridges to Wellness program allows me to do just that as a Community Health Worker!"

We are thankful to have Laura and her passion for serving others joining our Bridges to Wellness team. We are excited to see the difference she will make!!🤍⭐️

Bridges to Wellness spent the day at New Town Mall for the 25th annual reading festival.
