Do you know of an individual with dementia and their caregiver who could benefit from at least monthly contact with a care coordinator? We now offer two different service lines for persons with dementia. Our Dementia Support Complete program for individuals with commercial insurance or Medicare Advantage plans and CMS GUIDE Model program for individuals with traditional Medicare. We are here to help navigate the challenging journey of dementia.

Our very own Dr. Timothy Sigward spoke at the 2025 Elder Care Symposium organized by the Law Practice of Dennison Keller on the CMS GUIDE model program as well as participated in a Q&A session regarding dementia diagnosis. It was an impactful day of learning for all.
