If you have not returned your patient forms, you have not been established as a Patient with Panhandle Primary Care and unfortunately services will not be able to be rendered by Mrs. Russell. "Everyone" who requested to be an established patient has received a FORM REQUEST email from Elation Health with the necessary forms. Please see the below picture for an example of what the heading looks like once opened. Please email PanhandlePrimaryCare@gmail.com with any questions. Please note that the forms are not sent from PPC's email address, but rather Elation.

The outpouring of love and support since opening up my office two weeks ago has been overwhelming, THANK YOU.

For those patients who have already come over, welcome aboard, I look forward to serving you. To the patients who requested to become a new patient, by now you should have received an email with the forms necessary to become an established patient. Please fill out the electronic forms so that we can finalize your patient profile in our portal. If you requested to become a new patient and have not received the forms, please email Panhandleprimarycare@gmail.com with your name, date of birth, phone number, email address, mailing address, preferred pharmacy, and any known allergies.
