We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our community. The ideal candidate will possess a strong clinical background and demonstrate exceptional patient care skills. As an LPN, you will play a vital role in providing high-quality nursing care to residents. Your responsibilities will include administering medications, assisting with patient assessments, and collaborating with healthcare teams to ensure optimal patient outcomes.
Pay: $57,000.00 - $62,000.00 per year

Key Responsibilities
**Administer medications and treatments in accordance with physician orders and PATH policies
**Monitor residents’ health status and promptly report changes to the physician
**Maintain accurate medical records and documentation
**Support residents in managing chronic conditions and promoting healthy lifestyles
**Provide education to residents and staff regarding health and wellness topics
**Collaborate with clinical, therapeutic, and administrative staff to ensure quality care
**Maintain compliance with Vermont State and Facility Regulations
Requirements
**Current LPN license in the State of Vermont
**Experience in a therapeutic, behavioral health, or residential care setting preferredStrong communication, organizational, and teamwork skills
**Compassionate, patient, and adaptable in challenging situations
**CPR and First Aid certification (or willingness to obtain)
