Can you lift 50 lbs on a regular basis? According to occupational research, 50 lbs is the maximum most workers should perform on their own. If an object is heavier than this, this will usually require a 2 person lift. Please email Dr. Ibarreta if you have any questions regarding lifting requirements in your workplace.

Activity Prescription Forms (APF's)! This is probably the most important form your employer of injury, vocational counselor, and claims manager will want to see regularly if you are injured and are on disability. An APF is a summary of your medical condition and documents your current work restrictions, treatment plan, and future medical care. Make sure to keep asking your medical provider to provide an updated APF regularly. Also, double-check to make sure your lifting restrictions are correct. Has your timeloss stopped unexpectedly? Make sure you are filling out your work verification forms! These forms act as a double check for your claims manager to confirm that you have not been working. Your dates of disability need to coincide with the dates certified by your doctor on your completed activity prescription form (APF). I have provided English and Spanish links below:

Would you like to re-open your work injury claim? The Department of Labor and Industries will cover the cost of a doctor's visit for an examination. For medical-only benefits, there is no timeline limitation for re-opening. For full benefits (ie. timeloss, vocational services, impairment rating, pension, etc), the statute of limitations is 7 years after claim closure.
