Open Position: Certified Nursing Assistant (CNA) / Home Health Aide (HHA)
Reports to: Quality Care Manager
Position Summary:
Cares for individuals who may be disabled, chronically ill, or cognitively impaired. Helps older adults who need assistance. Supports patients by providing housekeeping and laundry services, shopping for food and other household requirements, preparing and serving meals and snacks, running errands. Monitors patients’ conditions by observing physical and mental illnesses, intake and output, and exercise. Functions under the direction, instruction, and supervision of the staff nurse

Position Responsibilities:
• Maintains a comfortable, orderly, safe, and clean environment for patients
• Dresses patients according to their needs, ranging from minimal assistance to totally dependent
• Bathes patients using tub, shower or provide a bed bath
• Prepares and serves meals and snacks as needed. May have to feed patients who are physically unable to feed themselves
• Toilet patient as needed; May also have to transport patients to the bathroom and provide incontinence care.
• Medication reminders as directed by the care plan, nurse, and/or physician
• Takes and records vitals
• Assists with Range of Motion (ROM) as directed in the care plan
• Ensures that non-ambulatory patients are turned regularly to ensure comfort and to help prevent bedsores
• Lifts, moves, and transports patients using proper body mechanics or lifting devices for accident prevention
• Reports all changes, including physical and mental, to supervisor
• Transports and accompanies patients to doctor’s offices, hospitals, and errands if necessary
• Complete and sends in daily documentation on the care provided to each patient concisely, accurately, and completely, in a timely matter, in the appropriate format, and on the appropriate forms
• Accepts reassignments as needed to provide necessary care for all patients

Minimum Knowledge, Skills, and Abilities Required:
• Ability to lift fifty (50) pounds. Moving, lifting, or transferring of patients may involve lifting up to 100 pounds
• Ability to exert maximum muscle force to lift, push, pull or carry patients
• Ability to use abdominal and lower back muscles over time without fatigue
• Ability to stand for extended periods
• Ability to bend, stoop, stretch, twist, sit and reach
• Fine motor skills
• Visual Acuity
• Performs position related duties as assigned, depending on the assignment setting
• High school graduate preferred
• Eighteen (18) years or older
• Able to read and write consistent with job requirements; cognitive skills related to the position, typically acquired with a high school education

Working Conditions:
Working hours include days, nights, weekends, and holidays. The number of patients assigned per shift may vary with the assignment. Work is physically demanding. Performs tasks that are unpleasant (emptying bedpans, changing soiled linens). Spends numerous hours standing and walking. Moves patients in/out of bed; Follow proper body mechanics and procedures for lifting/moving patients. Faces hazards from exposure to chemicals and infectious diseases. In addition, patients may be confused, irrational, agitated, or uncooperative.
