Medical, Urgent Care, Dental, Mental Health, Substance Use Disorder Treatment, Podiatry, Eye Care & Optometry, Optical Shops Community Health Connections (CHC) is a multi-site, non-profit health care center offering urgent care, primary family medical and pediatric care, preventative and restorative dental care, oral surgery, behavioral health services for children and adults, and substance use disorder treatment, and specialty services including optometry eye care, optical shop, acupuncture, nutrition consultations and podiatry. CHC has five sites within Fitchburg, Gardner and Leominster with decades of experience as a Federally Qualified Health Center (FQHC), serving 36 communities in North Central Massachusetts. All CHC health centers are located on the Montachusett Regional Transit Authority bus line. Community Health Connections was established in 2002 and is a federally qualified health center (FQHC). Community Health Connections (CHC) is a mainstay for compassionate care, regardless of income or insurance status. Our mission is to provide high quality, affordable health care in our community. Our vision is to be the leading health care center, committed to eliminating barriers to care and building a healthier tomorrow for the communities we serve; Acton, Ashburnham, Ashby, Athol, Ayer, Baldwinville, Barre, Boxborough, Clinton, Devens, East Templeton, Fitchburg, Gardner, Gilbertville/Hardwick, Groton, Harvard, Hubbardston, Lancaster, Leominster, Littleton, Lunenburg, New Braintree, Pepperell, Phillipston, Princeton, Orange, Royalston, Rutland, Shirley, Sterling, South Lancaster, Templeton, Townsend, Westminster, West Townsend, and Winchendon. Last week we held our 14th Annual Homeless Persons Memorial Breakfast at the Highland Baptist Church in Fitchburg. We gathered and paused to reflect and honor our friends and patients who lost their lives while living on the streets this year.

This year we honored the memory of 5 friends who are no longer with us. Their lives mattered and they are not forgotten. "Today was a day to remember, to be together and to honor our friends we lost while experiencing homelessness. I am thankful to everyone who helped provide a warm meal and essential supplies. This gathering reminds us of the power of community and the importance of continued support for those in need, not just today, but every day.
