Here at Fort Dodge Health & Rehabilitation, our talented and focused staff will work with you, your family, and your healthcare provider, to create a comprehensive and effective care and treatment plan. Here for you when you need physical, occupational or speech therapy following orthopedic surgery or an acute illness. We offer Skilled Nursing, Short-term Care and Long-term Care, and Physical Therapy
We also offer Additional Treatment Services including PT, OT, & ST, Outpatient therapy, and Hospice. We are conveniently located near Unity Point Hospital and only a few block North of downtown Fort Dodge. We have a stellar activity program. We bring in a variety of community, group therapy, outings, and so much more. We have a beautiful courtyard for residents to garden and sit outside which is perfect for relaxing! Services offered at the facility include: 24-Hour Concierge and Qualified Caregiver Assistance, Emergency Response System, Medication Management, Housekeeping Services, Linen Service, Transportation, Social programs, Outings, Special Events and Parties
