Retropulsion (a backward loss of balance) is a common postural instability pattern seen in individuals with Parkinson’s disease — especially during sit-to-stand and stand-to-sit transitions.
One contributing factor is insufficient forward weight shift, where the center of mass does not adequately translate anteriorly before sitting down. As a result, the body remains positioned too far back, increasing the risk of losing balance posteriorly.
In this session, we used an external cueing strategy: placing a chair in front of the patient and instructing her to reach forward and lightly touch the chair before sitting. This simple target helps facilitate anterior trunk translation and promotes safer, more controlled descent.
This is one of many therapeutic strategies that can be used to address retropulsion. Intervention should always be individualized based on the patient’s presentation, disease stage, and safety considerations.

✨ Laser Cane for Freezing of Gait (Parkinson’s)
This patient did not respond well to auditory external cues like a metronome — even with consistent rhythm, his freezing and shuffling persisted.
But when we switched to a visual external cue using the laser cane, his gait improved almost immediately:
💡 Increased step length
🚶‍♂️ Reduced freezing during turns
🔁 Smoother gait rhythm and initiation
👉 Important note:
Every person with Parkinson’s is unique.
Some respond better to auditory cues (metronome, counting, rhythmic music), others to visual cues (laser line, floor markers), and some need a combination. There is no one-size-fits-all — therapy should always be individualized.
Visual cueing works by giving the brain a clear external target to step toward, helping bypass the “motor block” that often triggers freezing episodes. For him, the laser cane made a meaningful difference.

Here’s what that means under current Texas law:

✅ No referral needed to start PT: You don’t have to wait for a doctor’s order to get evaluated and treated by a licensed physical therapist.
📅 Up to 30 days of care: You can receive PT treatment for up to 30 consecutive days without a referral — that’s powerful access to care if you’re hurting or injured.
💡 After the 30 days, a referral may be needed if treatment continues.

👉 Why this matters:
Starting PT early can reduce pain sooner, improve outcomes, and help you get back to the activities you love without unnecessary delays.

📍 Note: Insurance coverage sometimes still requires a referral even if the law doesn’t. Always check your plan — and we can help you understand what your insurance covers!
