Thank you to everyone who joined us for the Spring Learning Collaborative in Carbondale yesterday!

Chris McDowell (first photo), Executive Director for the Valley Health Alliance (VHA) welcomed CCA’s VHA Program members, kicking off a day of thoughtful discussion and shared learning.

Tom McCauley (second photo), CHRO for Aspen Valley Health, addressed staff burnout and retention. Then CCA’s Laura Palmberg, Clinical Improvement and Performance Analyst, and Amber Christie, Manager of Care Coordination (third and fourth photos) led a discussion on how transitions of care support continuity for rural patients.

Alice Harvey (last photo) with Home Key Health highlighted the power of collaboration in strengthening home-based care across the Tri-Counties, and Kaen Lapides, WMRHA Regional Health Connector, shared WMRHA’s Resource Guide. The day wrapped with a networking lunch where conversations and connections continued.
